Sorry you had a difficult voice experience.

In the beginning of June I was patient #15 in line for Dr. Thomas. I was told it would be 2021 until I had my surgery. Some may drop off or have different circumstances now.

I am in a year long serology study and had 3 blood draws for covid antibodies. When our IACUC department approves the study we will be notified of our antibody status. I am hoping that may make a difference to the doctor and positively influence when I am taken for surgery. Maybe it is me being optimistic.

Rachel:

One good thing with Dr. Thomas is that he does surgeries in an outpatient surgical facility; therefore, he is likely to be less impacted than surgeons who do surgeries in the hospital. Thinking good thoughts for you - I hope that you can get in sooner rather than later.

I myself am hoping to get out to Portland later this fall for an evaluation and to see if laser tuning would do any good. I still sense that I have some air leakage in my voice. The good thing is my voice can be plenty loud. It is still healing as well!

I think it is much easier to get an appointment for a consultation / outpatient procedure than it is for an actual surgery. The laser tuning is done in the office, and I got the sense from Jody that those types of appointments do not need as much advance notice. Honestly, my voice is more than fine at this point, so it’s not a high priority. I just want to see if there is an air leakage issue that can be repaired.

Alas, it is a really awkward time in my transition for me to be heading to Portland, as you can see in the timeline in my signature. Maybe I could squeeze it in during November? I’ll make a decision next month on whether I’ll try to get out there again...the stupid pandemic makes it really hard to plan anything that far out. I mean, my FFS is in three weeks, and I don’t even know if that procedure will actually happen.  >:(

Last month July 13 I was #11. Today,  8/7/2020,  I am #6. So 3.5 weeks 5 surgeries have been done or 1.4 per week. At that rate I should be in surgery in a month.  I asked Jody for a date so I can take off from work.

Jody emailed me and said there would be a pre-op visit and the op. There would need to be 2 trips. I e-mail back that I was already there once and at that time decided on FemLar. Anyone else go the Dr. Thomas and how many visits were required including surgery?

I think maybe she misspoke. If you've already been there once then that was your "first trip." At least that was my experience. Maybe she didn't realize you'd already had a consultation.

@Kwala, Jody called an hour ago while I was on my way home. She will call my case worker at Aetna and see if the 1st visit will satisfy for the per-approval.

I am now #1 on the list and the operation will be in October. I will finalize details tomorrow with respect to date.

I will need to have a covid test done 3 days prior to the operation at a site of their choosing and then be in seclusion until the operation. 2weeks no voice and then 25% increase per week. I am so excited.

I will be in Portland October 7 through 16 for vocal surgery. They have some strict covid requirements. I have to be there Oct 8, covid test on 10th or 11th, operation on the 13th, see them the 14, 15 and 16 and go home.

Dr. Thomas is in network and I am covered :)

When I called UNIM to apply for FMLA (needed where I work if out more than 5 days). They said Dr. Thomas's information was in their system because someone before me went to him.

 I pay $750 for overnight in the surgery canter. Next is plane tickets.

Assuming you're comfortable with Airbnb in the pandemic, get a place somewhere in the Nob Hill district, which is where Dr. Thomas office is. That's what I did, and it worked out great - I walked back and forth to the office no problem, even the day after surgery.

It's a beautiful neighborhood that is nice to walk around in, with plenty of shops and lots of great restaurants from which to grab takeout. There are several grocery stores near Burnside Ave. (the south side of the neighborhood), so if you want to cook and be close to a grocery store, find a place closer to Burnside.

Given the pandemic, there should be plenty of availability and prices should be more affordable than usual.

Oh, if you want to go to a great bakery, you must go to Ken's on 21st Avenue...they have a national reputation and it is fully deserved. I think I went there every other day for a treat...so good.

@sarahc, thank you for sharing the information.

With Covid I will arrive in Portland on the 7th early from Philadelphia. Shop for food for a week and sequester. On the 8th get a covid test at the place they require.  Sequester until Monday when I see the doctor at his office. Then sequester. Next operation and stay overnight as I am there alone. Then sequester and see them Wed-Friday.

Then, what happens it it goes horribly wrong? IDK, I think I get these thoughts because I have had 2 horrid experiences.

The negative experiences can definitely let doubts creep in, but I think the doubts come anyway. I totally had doubts before both VFS and FFS, especially since I had never had surgery before.

It’ll be interesting to see if I get doubts before GCS. There was definitely some ambivalence in my mind with VFS and FFS in terms of: “Is this really necessary?” But GCS is different: I have been wishing for GCS for decades, and I don’t think I’m going to get wishy-washy about it at the last moment. Nervous - yes. Doubts - hell no.

@sarahc, I had no doubts about GCS. I did ask myself if I was crazy. I think because I had FFS scheduled 2 months before GCS. I had two major surgeries in a two month period. I would ask myself are you sure and I would think would I pass or am I just doing too little too late. I scheduled the first of two hair transplant sessions after GCS.

GCS I knew I needed and from a young age. I was so relieved waking up post op. I was in the OR ICU for 1/2 day and then taken to the regular ICU for a day. There was an issue with extremely low potassium. They thought I had a heart attack. The heart specialist looked at my ecmo which was after 2 bags of potassium was given via IV and said my hears is fine.

My voice was pretty good before the subluxation. Post subluxation I had a glottoplasty. I would have been done but the doctor did not do the glottoplasty correctly. I got a second opinion and saw exactly the three issues.  I did not have to be a doctor to see the issues. Web way to high, vocal fold that needed to be debulked and two vibratory margins. He put the web in the middle of my vocal fold making two voices. I can not tell you how much of a PIA the lower opening is. I get fluid back in the bottom when I swallow.

Dr. Thomas would not comment. So I made the comments and asked if I was wrong. He said he could not comment. He gave me several options. 1) debulk, 2 web being taken down full or partial, 3) FemLar and 4) provide the trachea shave I should have had with the first operation.

I chose all 4. He explained that an infection would be very bad and why. He asked me to listen to I forget what recordings on his website to hear what happens when there is an infection. He said he has them on his web site to allow people to hear results when something goes wrong.

Hello Rachel,
I know it is easy to say, but on two of these, I think there is little to be worried about in fact. That is not to say, YOU shouldn't worry about them.

I have flown several times in the past few months, from San Diego to Chicago, Spokane, and a few times to PHL even. As one who traveled 40 weeks a year prior to COVID, I think flying is easier now than it was. The TSA lines are shorter, and depending on your airline, the airplanes are less full. (Although my last flight on AA from ORD to SAN was jam packed completely full.) So, on that point, you might find you like the travel part even better. As for the smoke, when I was in Spokane, it was VERY smokey. Quarter mile visibility, and smelled like grass fire. Me, my clothes, and even my mask all smelled like smoke until they were washed. But you still get through it. That said, Portland is in pretty good shape, air quality wise, these days. https://www.accuweather.com/en/us/portland/97209/air-quality-index/350473

On the other two, I hear you. But on travel and smoke, I do not think you will have any issues...
