As far as I understand and know (someone please correct me if I'm wrong), the vaginal opening is *always* below the labia minora, because if the surgeon were to extend the labia minora below the vaginal opening, it would most likely tear apart with the dilation regime required for the first year.
However, Dr. Suporn/Dr. Bank do offer a free revision after a year of the surgery (don't know if other surgeons offer a *free* revision), and I've read of many women who choose to have the labia minora extended below the vaginal opening.
