I'm writing to ask if anyone else has had a similar experience with this situation.

I'm mid-thirties, male-to-female, and generally healthy and fit. I transitioned with HRT and facial electrolysis starting seven years ago.

A year and a half ago, I had a double-orchiectomy. No complications were noted during the procedure, nor were any problems apparent in my post-surgery follow-up examination a month later. My HRT intake was immediately cut following the surgery as usual.

In the months that followed, I was only very occasionally sexually active. In the last year, I have become increasingly more sexually active.

Six months ago, I began to notice spots of blood in the front of my underwear following sexual activity. I've observed dribbling of bloodied translucent fluid from the urethra following sex, and I've even observed ejaculation of blood. I've had no other symptoms (no pain, no other unusual discharge, no fatigue), and blood is not visible in my urine.

I was tested for STIs and UTI six months ago and this month, and I was found to be clean both times. A simple internal-finger examination by my doctor indicates my prostate is in good health.

I was advised to avoid bicycling (I've commonly cycled a few blocks to a mile at a time around town on about a weekly basis my whole life) and any possible trauma to the genital and perineal area (I'm actually not into kink), and the problem should resolve in two weeks. It has not.

My primary care provider and clinic is now stumped, saying this is out of their range of knowledge. My network's urology department is apparently stumped, saying we've done all the testing they can do, and this kind of thing should have healed itself by now. I'm waiting to hear the opinion of my surgeon.

I'm worried this could be a complication of the surgery, that something didn't get sutured properly...but my PCP and the urology department are highly skeptical of that scenario.

Would anyone out there have had a similar situation and could offer their experience? Thank you very much.

I'm not a doctor, and I don't want to be Olivia Obvious, but you seem to have a lesion somewhere.  As it is related to sexual activity, to me that points to the urethra, Cowper's glands, prostate, seminal vesicles, or ductus deferens.

During sex, is your penis roughly handled?  I know on HRT, the MtF can develop thin skin, and some of the girls talk about bruising more easily.  I'm not sure if that would be related.

No rough handling of any specific area, and no bruising. Could I have pulled something simply doing sit-ups? And of course, I don't expect medical advice here, just hoping to find someone else who may have had a long-term case of hematospermia following an orchi, to hear their experience.

Have the same exact thing. I don't think doctor's even know what to do. No trauma, and being asexual, not worried about bruising due to rough sex.

Would love to know if they find anything out.

My doctor is sending me for a urinalysis and a sonogram of my abdominal area to include kidneys, and bladder. It seems like a shot-in-the-dark from what he was saying.

Well, I finally got my records transferred from my primary care clinic to my surgeon, and I was able to speak with my surgeon over the phone.
• He confirmed that it is his procedure to seal the vasa deferentia shut and to secure them in place, and he denied that they could ever possibly tear open or loose.
• He was very surprised that I would have this problem for so long, that it would happen every time, and that it would happen even after abstaining from sexual activity, bicycling, and exercise for two months.
• He also said a sonogram would reveal nothing, the detail would be too blurry.

So basically he's stumped too, he has no suggestions, and he's denying it could be a complication of his procedure.

Bit of a crazy longshot, as I know this thread is years dead, but my girlfriend is having this problem too, a little less than a year after orchi. Urine samples have blood, but otherwise it's not noticeable except in ejaculate (and that started quite some time after the orchi). Her GP and surgeon have no clear idea what's awry.

Did anyone in this thread figure out what was happening? If so, could they please point me to any relevant resources or pass on info for the doctors who know what this is?

It might b the same what I have.  A couple of years ago blood was discovered in my urine.  first they did an MRI of my kidneys, but could not find anything wrong there.  they decided t shove a scope into my bladder and found the problem.  My prostate is somehow sitting relatively funny, and is pushing into my bladder.  There the constant pressure cause two veins to swell up like varicose veins which leaked blood into the bladder.  I was put onto the strongest dose of Finasteride, and the problem disappeared.  I was told that the problem may come back, once I stop taking Finasteride.  Even after an orchiectomy I would still be required to take Finasteride!

I hope they find the problem

Spitvalling here....have the Doctor' ruled out renal calculi partially blocking the urethra? Kidney stones can be asymtomatic and cause UT bleeding.

I posted about what I think is the same thing (don't really have time to lookup the post link), I'm really glad this post got resurrected and to see that I'm not alone in this symptom.

I first saw blood in seminal fluid when I'd gotten to a year post-op and not having ograsmed to that date, decided it was time to expel accumulated semen by pressing on my prostate during play (something I'd done for fun when not wanting an orgasm in the past while wanting some release).

What I was surprised to see was a dark red-brown fluid on my fingers. I've had quite a few appointments to have it looked at and basically there's no explanation -- I'm in the diagnosis of hematospermia-unexplained and apparently not a problem. Prosatitis has been ruled out, my PSA is rock-bottom low as would be expected from someone who'd never had a high PSA result and has minimal testosterone in their blood, also no infections etc. I also have very slight traces of blood in urine, to be expected given urine passes thru the prostate / G spot.

So it's helpful to know that others are seeing this. It's not uncommon in the general population. I have to think that this is a part of the orchi where blood is leaking from the surgical site thru the vas to the prostate. Originally I'd guessed that it was there because I hadn't used the prostate in so long -- and I suppose that could still be part of it (still orgasm-free :-(). Discolored semen is a thing that happens when the prostate doesn't get flushed for long periods however what I see now is it's red-brown no matter what the period is between expressing semen manually. I'm going to contact my surgeon again now that I know others are seeing this.
