Study Result
Narrative
CT abdomen and pelvis with IV contrast

Comparison: None available..

Indication: lower abd midline pain chronic, ? abdominal wall, abd pain,
R10.9 Unspecified abdominal pain.

Technique: CT imaging was performed of the abdomen and pelvis following
the uncomplicated administration of intravenous contrast (Isovue-300, 150
mL). Iodinated contrast was used due to the indications for the
examination, to improve disease detection and further define anatomy.
Coronal and sagittal reformatted images were generated and reviewed.

Findings:
Lung bases are clear bilaterally.

The liver, spleen, pancreas and adrenal glands are unremarkable. Small
bowel is normal in course and caliber. No evidence of obstruction or focal
inflammatory change. No adenopathy. No free air or fluid collection. Normal
caliber abdominal aorta.

The kidneys are unremarkable without hydronephrosis or urolithiasis. No
findings to suggest appendicitis. The gallbladder is unremarkable. No
intrahepatic or extrahepatic biliary dilatation. Bladder is unremarkable.
Post surgical changes noted in the left groin. No soft tissue mass or
intra-abdominal fluid collection. No acute or suspicious osseous
abnormality.

Impression:
No CT findings to account for lower abdominal midline pain.

WHAT THE FUDGESICKLE POPPYCORN FRACKLESTICKS
They found 'nothing unusual'.  At all.   No mention whatsoever of the reproductive structures, or lack there of.
Apparently either they saw a uterus & ovaries (Pretty unlikely at this point as it'd be beneath the pelvic floor), or .. they're just plain frackin blind docs.  It does say female on the scan.

Why wouldn't they even mention it? Oh at least they noted they saw my left inguinal hernia!  Helloooooo.

Why why why why why lol.  Can you understand my frustration???? Or maybe I have one and its ... gah. Forget it every damn scan says something radically different, even if it says nothing at all.  Oh, I have the scans too btw. Jeez.

I know I mentioned it before but download Invaleus from the Brazilian government web site and load you dicom files in it. If you play with the filter you can view muscle and other soft tissues as well as blood vessels in 3d.

I'm sorry you still can't get someone to actually look for your problem.

They should have noted any reproductive irregularities... I had an abdominal CT scan done a couple  of weeks ago and they sure made note of mine! (I'm 20 years post-op)

PELVIC ORGANS:The urinary bladder, prostate, and seminal vesicles
appear normal. There is no free fluid or adenopathy. The patient has
had gender re-assignment surgery.

I think I would make a copy, put a big red F on it with note saying no discussion of reproductive organs then send it back to them. In the real world, ask the doctor to request they do another reading looking for reproductive issues.

I have a shadow in my right lung about the size of a hard ball. It has been in there from when I was a child and is the result of valley fever. The X-ray lab wrote up a report that I had serious issues so I had the doctor ask them to pull up a previous X-ray and compare it with the current one. The next lab report came back clean and I am still alive many years latter.

I actually did just that. Sent it back for reevaluation.  Its kinda strange they 'skipped over' the reproductive organs and said I was perfectly normal. Especially since that was the chief complaint.  The tech asked me too, and I stated I had an unidentified organ behind the bladder below the pelvic wall.  I couldn't have been more specific than that.

And ya they didn't make any note..whatsoever.   Odd to say the least.  I mean state things one way or another. Or state there is nothing.

So its been a week since I requested clarification on the CT scan.  No word from either the radiology department or the primary (reproductive endo that ordered it allegedly).

Should I bug them again?  How soon is too soon?  Surely a week is long enough? I hate this wait game they all play.  It hurts daily, and that doesn't seem to bother anyone but me. Feels like my insides are twisting & cramping all the time. I don't want to live on muscle relaxants.

I don't know Doreen.  ???

I was just looking at your imigur pictures again. Do you have any sagitial views?
By my very amature radiology skills, mostly comparing what I can see in your compared to having looked through my scan slices, is that you have something that looks different sitting above the bladder slightly. It's definitely not large intestine so far as I can tell.
The top view shows two diverging looking structures behind the bladder. So in my "normal" male inside organ slides, there is one set of diverging structures which are my seminal vesicles. Also since my CT was long before starting HRT mine were still full sized. This makes me doubt that anything in your scans is seminal vesicles. Just based on what you have said before and since you obviously did not have a male reproductive system when you just had this scan done.

I don't know. I have learned a lot by going though my slices and looking online for comparisons. The only thing I can tell is you are different girl. I'd be calling the doctors office and asking them to check on the re-reading of the scans.

I found the radiologists email that did the initial read and asked for clarification.  He hasn't responded (yet) but apparently my reproductive endo (the primary there) talked to him, and they are still reviewing it.

Noones suggested I have seminal vesicles that I know of yet. Or a prostate.   I did have one radiologist (local) that went off of what a pediatrician said that they 'might' be those structures, just very infantile apparently.  Who knows.  The structure behind my bladder is triangular in nature and very similar in size to an infantile uterus too... just lower than expected.

I don't know.   My best guest is since he's taking his sweet time getting back to me, I'm theorizing the radiologist is seeking other opinions too.  At least I hope.  Simply ignoring the structure isn't an option.. pretty sure that's where my constant issues are coming from at this point.

Hopefully I'll get more information from the ob/gyn that specializes in mullerian anomalies.. or from the radiologist after re-reviewing it.

It does look like there is something there. I hope they can find an answer for you.
Honestly I would have no idea except to think maybe a uterine/Mullerian structure.

I had sent some of the images to another radiologist, one that deals specifically with intersexed issues, and she called it a 'uterine structure'.   I  think I'll take her word on it over other radiologists who frankly don't.  That being said I need & want something definitive finally.  I'll find out...some day.
