I also thought about SRS with Dr. Sanguan in Phuket Thailand.
Apparently he has worked with Dr. Suporn for quite some time and uses a quite similar approach to SRS. He also uses a full graft technique - butr in a different way.
He performs SRS in two stages while your one visit to Phuket. During the first stage he prepares the vaginal tunnel,  removes the penis (Penectomy) and the testes (Orchidectomy), constructs the  clitoris, clitoral hood, the labia minora (Labiaplasty) and construction the new urethral opening (Urethroplasty). He then attaches a special vacuum assisted device so as to enhance granulation tissue growth, promoting healing, and preventing swelling. He believes that his method is the best one to maximize graft attachment and graft survival.

I have been informed that Dr. Sanguan doesn't use epidural block anesthesia any more - or only on request of the patient. Actually the epidural anesthesia still prresent on the website text is something they don't do anymore.
Funny enough this was the reason why I didn't choose Dr. Sanguan for my SRS plans any further.

Also the website still says that he uses penile inversion and labiaplasty - which is simply incorrect. Apparently he informed his web host to implement the changes of content on the website.

Should you be interested just email him and he will explain the current procedure in detail.

After SRS response is also very fast - so communication with Dr. Sanguan is very, very good.

Anyway - he is a really, really great alternative to Dr. Chettawut and Dr. Suporn - his SRS prizing ranges in between the other two surgeons and his results seem very, very good and natural to me.

Overall I do like his procedure A LOT NOW hat epidural anesthesia is not standard procedure anymore.
You will find loads of very, very favorable experiences from the "Sanguanistas" in different Yahoo groups - or maybe even here?!?

He does still use the epidural block on the first surgery you do have a choice  but not the second

He is a fab surgeon and I'm very happy with my results

My surgery is scheduled for Tuesday 3rd Feb, about a week and a half away. I did a lot of research and I am very happy with my choice.
What are the pros and cons of having epidural? i assume that the alternative is a general.

The epidural decreases the amount of general anesthesia that is used during surgery - and apparently it minimizes side effects like nausea after surgery.
The risk with epidurals is permanent nerve damage.
Epidurals achieved bad reputation in Germany because some op-gyns use it for pain relief during child birth.
Yet if a woman has permanent nerve damage afterwards she wishes she would have never done it - and we had quite some cases. Just think about never being able to climax - or not being able to control you pelvic muscles....

I just emailed Dr. Sanguan 5 day ago about this topic - and he replied to me that they do not use epidurals as standard procedure at the moment.

Whichever you chose - make it clear during consultation what you do want in terms of pain management.

Did Dr. Sanguan perform the trachea shave with the incision right above the Adams Apple or did he tunnel down from an incision on the neck crease?

(I'd been wondering why you were so worried about the epidural... and I think it might be a cultural difference! Here in the US *most* women who seek pain relief in childbirth will get an epidural, and the rate of complications is very, very low. So there are clearly a lot of very experienced anesthesiologists out there who can do an epidural with almost zero risk. I actually didn't think to check whether this was also true in Canada, but I think so, as the epidural was presented as much less risky than a general - and the one girl while I was there who had to have a general b/c of previous spinal surgery had all sorts of nasty side effects, including vomiting so much for days that she almost landed back in the hospital. I had no aftereffects of the epidural at all, not even soreness at the injection site.)

He done it in s natural crease on top x
