Hello Everyone

You may recall that in May and August and November I compiled previous tables on BT Results.

So once again I invite anyone that wishes to provide with their E and T Blood Test results after either 3/4 months or after 6/7 months or after 9/10 months or after 12/13 months, please do so on this thread please and I shall happily add you to the table(s). Please state whether you are providing International or US Units and I shall happily convert accordingly as the table provides both for comparison purposes.

I am doing this exercise in May, August and November 2018 and February 2019 just to see if there are any statistical trends we may see; I accept we may not see anything more than coincidence especially since I do not possess (and I do not wish to possess) details of dosage. Anyone may inform me but only if they wish of their medication name but not dosage of course. I do not wish any table to be regarded in any way as "League" or "Performance" table.

The previous tables are as above and the current table is as follows:

Jessica has just provided her BT results as below which I shall add to the tables in due course.

My 3 month BT was
E= <50 pg/mL
T= 1197 ng/dL

3 months later
E= 101 pg/mL
T= 21.6 ng/dL

5 months later
E= 92 pg/mL
T= 72 ng/dL

6 months later (most recent)
E= 122 pg/mL
T= 21 ng/dL

Thank you very much for agreeing to post your E&T results next week including 2 years retrospection. I am most grateful for that.

I hope you don't mind me asking but on the "Periodlike Symptoms" thread you stated your T in nmol as expected for Australia (or UK) but you stated your E in pg/ml (US system) instead of the customary pmol. Did you do the conversion yourself from Intl to US as per "Unit Conversions" thread attached to replies to this thread on Feb 7th or is your E of 163 in fact in pmol please?

Gosh, Katie that is a high 13 month E reading at 1483  - almost as high as my 6 month E reading of 1662!

Did you see my message above dated February 19th please?

On February 10th on the previous "Comparison" thread you said you would be providing 2 years restrospective BT results - are you now prepared to do so please? If you prefer to maintain the privacy and not provide them, I understand completely.

On February 8th on the "Period-like Symptoms" thread you provided one result in Intl units but the other in US units and I queried it but I assume the figures are correct as you are familiar with methods of conversion.

Thanking you for all your information and if you do not wish to provide previous results, I repeat that is fine. If you do wish to provide them, this is the thread for them please.

Oestradiol: 498 pmol/L = 135.66 pg/ml
Testosterone: 1.9 nmol/l = 54.76 ng/ml
Progesterone:  14.9 nmol/l = 4.69 ng/ml
Prolactin:283 pmol/l = 6.51 ug/l


Jan 2018
Oestradiol: 403 pmol/l = 109.78 pg/ml
Testosterone: 1.8 nmol/l= 51.87 ng/ml
Progesterone (Not tested as only started Feb 2018)
Prolactin: 278 pmol/l = 6.39 ug/l

Dec 2016
Oestradiol: 605 pmol/l = 164.81 pg/ml
Testosterone: 2.1 nmol/l =60.52 ng/ml
Progesterone (Not tested as only started Progesterone Feb 2018)
Prolactin (Not tested before Jan 2018)

PS One interesting thing as that since starting on Progesterone my Testosterone has increased a little. My endo believes that this might be because I had taken my blood tests during the combined cycle (I take Oestradiol only from Day 1 to Day 15 (Twice a day), then from Day 16 to 25 I take Oestradiol once a day and Progesterone once per night), then converting back on Day 26 to  Day 30 Oestradiol only (once per day). This is designed to mirror a CIS Gender female natural hormonal cycle. When I first started taking Progesterone I commenced on a a permanent  Oestardiol and Progesterone (Promethium) regime, but I found it too much. Now I find myself  feeling overall much better balanced on the monthly cycle, although I do find that I get a bit moody and weepy on or about Day14-15 per month. This is my signal to start on the combined regime. The regime for the increase is that Progesterone can be  effecting the Testostorone level slightly

Hi JudithLynn,

                      The cycling HRT regimen interests me. I have heard this method is better for feminisation including breast development. Do you think the addition of progesterone makes a difference ?

  Your experience has me intrigued! Would you ever use a standard Oestrodiol regimen ?

Perhaps we may meet with all UK girls in Autumn or on some future date; I am going fulltime in Summer.

Gots a few more since I last gave them. Last two would be at 21 months and 2 years.

 I'm concerned with the  estradiol being low again.  T a little higher than before but still within range. Haven't heard from my doctor yet, tests yesterday and posted in my chart overnight.

See my other post about Micronised Progesterone, but yes I was on an Oestradiol  only- Progynova regime for my first 4 years, initially one tablet once a day then eventually two tablets (one at 10am and one at 10pm) daily.

Obviously I cannot decribe dosages, but suffice to say that I started off on a very low dose and am now on a full strength (transition) dose.

Hi again Judithlynn,

 I also find full stength transitioning dose is better for general peace of mind and motivation. Having breasts and a semblance of a CIS womans figure helps a lot as well . I will trial micronised P- the feminising effects are too hard to ignore!

Hi all,
New to posting, but I guess this finally drew me out.  I'm 43 and have been on EV injections only the last 3 months, and on a patch for 25 days before that.  I switched due to skin irritation and because I prefer just having to remember an injection every other Sunday.  No androgen blockers or others drugs for now,  I wanted to see how E only went before adding in Spiro... as I had low E for a male starting out (around 150).  Readings in US measurements exactly a week between injections:
3+ months:
E (418)
T (15)

My doctor and I are pretty happy with these results so far, so going to continue with E only to my 6 month mark.

I am in the US: T is 17 ng/dl, but my E is 84 pg/ml

0-month: 286 ng/dL
3-month: 59.9 ng/dL

0-month: 33.2pg/mL
3-month: 28.7pg/mL (yes, it went down >:( - however, this test was taken at the end of a 12-hour dosing period, i.e., just before my next dose. Moreover, I'm decently happy with the 3-month physical results.)

Hi Pamela,
Got my 6 month results about a week ago... switched over to Kaiser now, so at least they are free.  Quest was killing me out of pocket.

Estradiol: 485 pg/ml
Testosterone:  <13 ng/dL

Both of my doctors are now advising me to lower doses about 20%, trying to get down to a little more like 200ish... but we'll see what T does.  I'm on EV injections only.

I got my test results to today, taken about month after my orchi  The LabCorp computer must have been wondering what was going on, cause it put the typical female comparisons to my tests results!
I have 1.5 ng/dl of Dihydrotestosterone, and I am low in that, a typical adult female has 4 - 22 ng/dl
I have 5 ng/dl of testosterone, serum, typical would be 3 - 41 g/dl
I have 139. pg/ml Estradiol and am in the ovulation phase of a female which would be 85.8 - 498.0 22 pg/ml

I do think I have to watch out to not get to close with men, because I don't want to get pregnant, being in the ovulation phase and all!

Bumping me up to the double dose of E basically also doubled my Estradiol levels in my blood.  I wonder if it would go up by 50% now if I would add 1/3 more dose during the day?
What if I would need to make the decision between the same amount of estrogen with added progesterone, or no progesterone and more estrogen?

Testosterone will not play any role anymore, considering that I am even low for natal female conditions.  That seems all my adrenal glans seem to be able to produce, fine with me!
I am really wondering what else the orchi will do for me besides giving me low T and the desire to wear skirts and dresses instead of pants?
My boobs seem to still grow with the same speed they did for several years now (watching grass grow is more exciting and faster paced), they still hurt as they did for quite some time now.  At least I can get some mileage out of the bras I am buying!

Your levels after the orchi seem different to me.  I have ~5 T, and my endo keeps me on ~140 E (which is the average level of a natal women on day 10 of her ovulation period - I should be a hottie, shouldn't I?)

the day after orchi my endo had cut my dose by of estradiol by 75% , I was on a very high dose pre-orchi to try and suppress the T. 
Had my appointment with her today and again we are going start to increase the dose until its get to 150 that's where she wants me.
     I understand decreasing after the orchi but I felt it was too much and as my E level shows im dragging around without much energy, but im not a doctor and followed her orders on this.
   As the level increase I will feel better.
I had really hard time suppressing the T the results I posted were the closest to Pamela's time frame but I actually had blood draw every 5 to 6 weeks there were times my T skyrocketed to 800 fighting back while trying to get it down. She wanted my numbers where your are but there is only so much you can do . In the end I won with the orchi

It seems that your endo has the same idea as mine E at 140 to 150.  But how come that your T is that high?  My levels were taken about four weeks after my orchi, and my T was only ~5 at that time.  Is there still some T roaming around inside you that will go away in the near future?  I hope so!  Life without T is just peachy!

The adrenal gland produces some T , and that is what is measured in my 5.  But 13 should be OK, too.  I hope they get you up to the desired E level soon.  I was very tired and lethargic shortly after the orchi.  My endo  doubled my E a week after it, and that brought me nicely to 140.

Hello Everyone

You may recall that in May and August and November of 2018 and February 2019 I compiled tables of E and T Blood Test results after 3/4 months and after 6/7 months and after 9/10 months and after 12/13 months.

Since February 2019 I have been using this thread to incorporate any E and T Blood Test results. Just to let you know that I do not propose to continue producing tables for 15/16 months, 18/19 months etc. If anyone else wishes to produce future tables they are welcome to do so of course and have my blessing!

However anyone may still provide their E and T Blood Test results after ANY PERIOD OF TIME by all means ON THIS THREAD PLEASE if they wish for comparison purposes. I would be very grateful if those that choose to do so would confirm either Intl Units used in UK, Europe, Canada, Australia or US Units please. Obviously I am a Brit but I appreciate most members are my friends from US.

I shall periodically visit this thread and convert Intl to US and US to Intl unless the provider has already done so.

May I take this opportunity of thanking everyone who participated in this Comparison Exercise since May 2018 and thank you also to those who will voluntarily continue to provide their E and T Blood Test results now and in the future.

Further to my message above I just thought I would list all my E&T readings in one place as follows:

As I say anyone may continue to provide their E and T Blood Test Results after ANY PERIOD OF TIME by all means ON THIS THREAD PLEASE if they wish for comparison purposes.

That is the general level my endo tries to keep me at, E between  140 and 160.  The last time I was at 140, but my T was also less than 5!
He wants to do blood tests every 6 month only, because after my orchi he does not expect any major changes in my levels.

My 9 month results are baffling.  Dropped my EV weekly injections by 20% the last 3 months.  My E levels are now even higher!  I’m taking Micronized P as well now in the last month,  so maybe it’s that.  Again not taking any AA-blockers.

9 months:
T = 13 ng/dL,  stable
E: 658 pg/ml,  previously in 400s when we decided to lower dosage 20% from what was considered a starter dose.

Well looking at these tables my T at0.5 nmol/L looks fine but oestradiol is low at 205 pmol/L.  When I used some extra patches (smallest) it went up to over 300

Curious, since you’ve had similarly high levels to me and are much further down the road,  have you been happy with feminization results?  I’m worried that having my levels so high could be counterproductive.

I had breast buds early and genital reduction early.

However I did not notice real breast growth till after 8/9 months. Hence I expect and hope you may see similar starting about now. I am content with my feminization and went fulltime Pamela June 29th.

I have heard before your point of high E levels being counterproductive but mine were only high at one reading and I think that was a timing issue (having BTs too soon after applying patches).

Hello Everyone

You may recall that in May and August and November of 2018 and February 2019 I compiled tables of E and T Blood Test results after 3/4 months and after 6/7 months and after 9/10 months and after 12/13 months.

Since February 2019 I have been using this thread to incorporate any E and T Blood Test results. Just to let you know that I do not propose to continue producing tables for 15/16 months, 18/19 months etc. If anyone else wishes to produce future tables they are welcome to do so of course and have my blessing!

However anyone may still provide their E and T Blood Test results after ANY PERIOD OF TIME by all means ON THIS THREAD PLEASE if they wish for comparison purposes. I would be very grateful if those that choose to do so would confirm either Intl Units used in UK, Europe, Canada, Australia or US Units please. Obviously I am a Brit but I appreciate most members are my friends from US.

I shall periodically visit this thread and convert Intl to US and US to Intl unless the provider has already done so.

May I take this opportunity of thanking everyone who participated in this Comparison Exercise since May 2018 and thank you also to those who will voluntarily continue to provide their E and T Blood Test results now and in the future.

Well we are very similar. My dose was increased by a third wef June 10th and my next BT Sept 2nd.

Just to add that I started Progesterone March 17th and I am wondering whether the Progesterone was partly responsible for the decrease in E from 558 (Intl) to 242 (Intl).

I'd had a problem getting my Spiro for a bit. I missed 2 days before I got it back, & my appointment was 2-3 days after that, so that may account for the high T.

I also just started Finasteride, & she said that should help decrease the T as well, so she'll take it again at my next appointment.

I'm not on HRT yet but I plan to have my levels measured before I start HRT to have a baseline. I'm quite curious because certain things indicate that my hormone levels are not really in the male range even without HRT. Would publishing those results be useful in this thread?

219 (Intl) is a bit low but please give it time. My last one of 242 (Intl) 66 (US) was also a little low.

My Estradiol levels have been kind of a struggle.
Timeline:
07Nov 2018 Estradiol start- two week ramp up. Daily dose week 3. No change in dosage until July 2019.

29Jul2019        First injection      90 minutes after shot, I got the same level of ..."calm" is the only way I can describe it that I remembered feeling in December of 2018. Two days after Shot and starting Progesterone my body was giving indications that it was in a development phase again (ie my chest hurt like heck and was oh my gosh sensitive). The ache settled down a bit after 4 days though the sensitivity is still there.

Sorry, I don't know what those values are in "intl".

While I am sad to see others having issues with consistency, I am kinda glad I am not unique in this situation.

I had an estrogen test done today, because of the orchi they dont test for testosteron any more.  This was ordered by the endocrinologist of the Mayo Clinic transgender department.  He had the feeling that idid not have enough when I told him the dose my very disliked by me endo wants me to take.
Anyway, he was right,  I only have 129 pg/mg.  That also explains why I feel realy slugish!

I have to see with what he comes up with, depending on my other blood values.

mine have dipped again, but nonetheless my chest is still growing, just into C cup now which seems to be quite good.

Serum testosterone level < 0.2 nmol/L [8.3 - 27.8]

Serum oestradiol level 243 pmol/L [41.0 - 159.0]

(expected cis female range in brackets)

I expect that my dose will be upped again.

This drop may be due to the face I was quite ill last week, loosing 7lb in 3 days (probably mostly water/waste though, and has since mostly gone back on)

I am sorry to hear you have been ill recently and that just like me you have had a decreased E reading.

Your E reading after 19 months of 243 (Intl) 66 (US) is so close to mine after 16 months of 242 (Intl) 66 (US).

Your T reading of 0.2 (Intl) 6 (US) is better than mine of 0.4 (Intl) 12 (US).

My next BTs are also after 19 Months on Monday Sept 2nd.

The endo at the Mayo Clinic gender department told me that they usually try to keep trans women on a rather high estrogen level for the first three years to support breast formation/growth, etc.
Because of my a little advanced age, he wants to wait until all the tests results are in (blood tests, ekg, etc.) before he makes the decision on how high he wants my estrogen level to be.  I hope I will hear from him soon, but still have to do another test on August 16.  He thinks that I might gain another one or two breast sizes if my estrogen level would be higher.  One size would be Ok with me, that would bring me to a very solid 40 C (US) cup, and I don't want much more.  I think my current boobs fit pretty well to the shape of my body, and that is all what I want.

It appears they follow the same practice of higher target range of E here in UK also.

As you Katie note, the normal post menopausal E range is 41-159 (Intl) which is 11-43 (US)
                         whereas GenderGP target is E range of 300-800 (Intl) which is 82-218 (US)

This is a most significant point which I do not recall being mentioned before on HRT Board but perhaps long time Members may have previously seen and noted this.

My current endo is so scared of DVT that he did not want me to be higher than about 150 US!
I hope the Mayo endo will get me a little higher!  I don't feel any influence of E and my boobs grow with the same rate as they did prior to me starting HRT!  Grass grows way faster!

Today I got the results from the lab to determine my hormone levels before HRT and it seems that because my E levels were lower than the levels for post-menopausal women and I was listed as a man, they did not include the exact E levels:

LH7.44 U/L
FSH6.05 U/L
P1.2 nmol/L
E<73,4 pmol/L
T15.2 nmol/L
Prolactin175.0 mIU/L
DHEAS   12.0  μmol/L

Am I right in thinking that these levels show a similar pattern to what is described as Primary (testicular) hypogonadism on the Laboratory evaluation of male hypogonadism image on the following page?

Hi Pamela,

My doctor's nurse hasn't put my latest labs in the patient portal yet, so I don't know what kind of measurement they use, but my new numbers are in after being on hormones for exactly one month. She called and said that my estrogen is 545 and that my T was 19. I notice you always have some conversion for INTL and US, but I have no idea how to do that.

Anyway do those numbers sound like they're good? My doctor said she always tries to keep her MTF patients between 550 and 950 with a T level below 50. So I'm perfect on the T and just a tiny bit short on my E. She is keeping me on the injection dose she started me out on for another two months. After that she'll check my labs again.

So that's my update. I look forward to hearing your thoughts, because I know you're the expert around here.  :)

Please forgive me but as you may have noticed the results on this thread are virtually all AFTER some period of HRT.

In my experience the majority of doctors and endos only ask for E&T readings after some period of HRT although before starting HRT they usually seek many other blood tests. Therefore I cannot comment on your readings of E 73.4 (Intl) 20 (US) and T 15.2 (Intl) 439 (US).

With regard to possible Klinefelter, I am familiar with the term but I suggest you wait to see if KayXo or Linde have any comment on that. They visit HRT Board quite often. However I think it is really a matter for you to discuss with your doctor. I wish you success with that discussion.

I am sorry I cannot give you a more favourable reply on your two points of concern.

I hope you are able to provide your E&T results after 3 months in November and then we can judge whether they are reasonable/expected or not.

Please forgive me but as you may have noticed the results on this thread are virtually all after at least 2 or 3 Months HRT and secondly I am on E Patches as opposed to Injections and the results vary between method and timing of BT.

That being said, your T reading of 0.7 (Intl) 19 (US) looks fine and you are well on your way to suppressing it. Your E reading of 2001 (Intl) 545 (US) seems extremely high.

I know you are from Indiana and hence assume the readings you provide are US readings. However if your provided readings are for some reason Intl (as used in Canada?), then your E reading of 545 looks reasonable.

I suggest you e-mail your doctor to verify the units please. US are pg/ml for E and ng/dl for T.   

By all means please continue to provide all your E&T results; are the next two after 2 months and then after 3 months please? Thanking you

So as was obvious really your provided readings are US units.

Therefore your T reading of 19 (US) 0.7 (Intl) is fine and your E reading of 545 (US) 2001 (Intl) is extremely high.

My own highest ever reading was 453 (US) 1662 (Intl) and that reading along with a few other Member's high readings prompted quite a few comments!

Now apart from studying our E&T BT results over the last 18 months, I have just recently been studying Expected Cisfemale E&T BT Ranges. I am surprised at how much the Ranges vary between different clinics/laboratories just within UK and I am sure that applies to US also. However I am NOT surprised that our trans Targets differ significantly from Cisfemale Ranges. I have as recently as Sunday 11th sent out to my 4 UK friends and my 2 Australian friends a PM on this subject. I shall now PM to you and you can read my message and their replies if you wish in your own time.

As you are happy with your doctor, all we can do is await your BT results after 2 months and 3 months and see if E remains high or stabilizes. I am surprised at your doctors trans Targets (as opposed to cisfemale Ranges) but it is not for me to judge any doctors' views.

I hope your T remains the same or reduces slightly and that your E stabilizes after 3 months.

As with Lynne, I think in the next week or so other Members may see this thread and comment on your early readings.

I had similar levels as well for 9 months and both of my doctors agreed it should come down to sub 200...  I’ve cut injections 40%,  find out in a few weeks if it did anything.  I’m way below the “starting” dose.

I don't know whether this is of any help, but the lab test result forms I got listed the maximum normal luteinizing hormone and follicle stimulating hormone values for men to be 12 IU/l. I gather the values may vary some according to the laboratory and test protocols used

The testosterone range was listed as 12-45 nmol/l, so assuming similar protocols yours would seem to be at the low end...

However, my result sheet also listed the maximum male E2 value as 0.15 nmol/l, suggesting the protocols/reference values may differ.

My doctor also suspected Klinefelter's—but the karyotype test showed XY. That made me sad, despite the risks it would have entailed.

I was informed to not give any medical advise or write anything that could be considered as some form of diagnosis on the public part of this forum.
she can contact me through a PM, and we can discuss any possible influence of intersex.
I was recently for some days of evaluation at the Mayo Clinic, and they told me that the never came across an intersex person with my symptoms.   They feel that I have a combination of several intersex symdromes, and some mutations on top of this. As a medical researcher, I consider myself the biggest medical experiment I was ever involved in.

Most results here are for periods of at least 3 months and hence I would be most grateful for your 3 month result in September please.

However your T reading 8 (US) 0.3 (Intl) is remarkable and hence it is virtually suppressed. Your E reading of 94 (US) 345 (Intl) also shows a good start.

On my lab results the max. reference value for LH was 7.6, so that level is on the higher end of their scale.

Interesting, T is staying pretty rock bottom, still not getting E down to desired results that both my doctors recommend (~200), so I imagine I will be lowering my dose further. I think that wraps up my hormone posting with basically a year mark.  Just to show numbers aren’t everything for those of you worrying about it... hormonally, I had a fairly “ideal” response, but you’d be hard pressed to know it looking at me, outside of slightly more paunchy man boobs. 🤪

Your E seems to be really high, I hope this will not cause any negative results.  I am now dealing with 4 different endos, and all are of the opinion that 150 to 200 US should be the amount one should have.

I thought it may be useful to have my pre-estrodial HRT levels on here as well, as I am a high-aromatiser for testosterone. As far as I am aware (as told to me by my endo) this is a fairly rare genetic trait, but just in case someone else has issues with "strange" test results it may help point them in the right direction.
I was on a "normal" male dose of testosterone at the time (and it was killing me!):

Oestrodiol  164 pmol/L
Testosterone 17.4 nmol/L

As you can see, my oestrodiol treatment now is designed to reduce T and E2 to post-menopausal female levels.

I just switched from pills to injections, and the amount of E was kept the same for the injections as I had with the pills.  It will be interesting to see, if my body gets measurable more E than it got from the pills that I took sublingual!
My Mayo Clinic endo feels that sublingual pills will not even come close to the injections.  I wonder?

I too am wondering if my dose will increase. My OBGYN is on vacation until the 15th. If I am 104 pg/ml at day 7 after injection, I wonder what day 1 levels are and do they make up for the low reading by the end of the week?

If "Sore boob is more boob" then my levels seem to be high enough to be promoting chest development. The day after my first shot (I switched from oral after 9.5 months)  it felt like they were trying to pull my ribs out for two days. Thankfully that did not happen as bad in the following weeks and now I am just very sore for 4 days and tender for the last 3.

I got permission  to drop the Spiro and the Dutasteride if my T levels were good (and they are beyond good) so I am off those two drugs right now  :icon_joy:. If I can keep my T level 60 ng/dL or less, I am happy personally. I am OK with a bit of functionality. If I wasn't I would aim to keep it in the 10 to 20 ng/dL range.
Hopefully the Spiro did not interfere with breast development and has not caused premature nipple plate fusion which is a current theory. Please note: Theory. There is documented evidence that transgender people who used Spiro are more likely to seek out BA surgery, which I am attempting quite hard to avoid.
With the T levels so low the Dutasteride is redundant: basically no benefit and all the chance of the side effects. pfft!

I will update if my dosage increases or not. Being the greedy "I want it now" bitch that I am: I hope so :D

Good morning,
On June 12th, my T measured 395 US. That is the day I started HRT.
At 3 months here's the results taken 2 hours after sublingual estradiol, on Sept 11.

T 140 nge1. This is less than half it was on day 1. Without any AAs.

E total 1480 pg/mL. This looks extremely  high, will ask if typo.called, they are redoing the test, something about using the wrong sex.
E1, Estrone, still awaiting results. Will add once I have it.ets to have on 9/19/19.
E2, Estradiol  142.9 pg/mL
E3, Estriol 0.1 ng/mL. I wonder if that is normal for males, would have thought it would be 0.0

Tested through walkinlab dot com using Labcorp.
Was less costly to get all four E labs as package, vrs total e and e1 ane e2.

My OBGYN is back from vacation. She has moved me to the "max dose" she is allowed. I was expecting doubling the dosage....Nope! I got a 25% increase. While I am not a medical person, I do understand enough math to realize that this pittance is not going to get my day 6 trough level to over 200 US. I think I will start shopping for new medical assistance once I have the employment situation resolved.

I have taken the Spiro and the Dutasteride off the menu however. I am attempting to use Progesterone as an AA. Due to how I am taking it, 95% is not being filtered out by the liver, so I am getting 20x the Progesterone into my body. Best sleep I have ever had in my life. I just have to make sure I allot at least 7 hours, otherwise I have a tendency to ignore alarm clocks. I has also made me hungry, so I am paying attention to how much I eat and how often.
Also, I have started taking vitamin C which, according to the Mayo clinic, taking vitamin C with oral contraceptive or hormone replacement therapy might increase estrogen levels.

Please note that vitamin C can have negative effects of other drugs. Do your research please.

So, yeah. Frustrated currently. The only thing saving my mental state is my chest is still very tender which implies that development is still in progress.

Sorry to hear that your OBGYN has a maximum dose that may not be adequate for you.

However you never can predict E results (you have probably seen how erratic mine are but at least I am now at the "right" E level of 819 (Intl) 223 (US) and hence no increase necessary).

Also it is surprising that so many doctors and endos and OBGYNs both in UK and US suggest different times after methods of medication to take BT. Some seek mid level and some seek low level of E.

When is your next BT due? I wish you good luck with your next test.

I am just back from a long stance with the Mayo.  Was turned inside out by them, and upside down.  The mayo has two endos for the gender department, and both looked at me.  They decided to take me off of the pill and put me on injections, because they want to increase my E levels (currently between 123 to 149 US), to a bit over 200.  They did not mention any vitamin C, and could care less about the progesterone I take.  Their major concern is my rather high prolactin levels, which should not be there.  But my pituitary gland is messed up anyway, and has never in my life functioned properly.
Their entire program with me is geared to get me ready for my GRS  in a few months.

There were several things that had to be done on my new house which have now been cleared. Hence I am quite settled in here now. I am almost 12 weeks fulltime and gaining confidence all the time. I have changed my name by Deed Poll to Pamela Wendy W. I have not had any mocking or trouble (apart from one insult which was before fulltime anyway). I suspect many in the neighbourhood know I am trans but they appear to be indifferent. I am so so much happier than I was. The 3 best things I did in my life were seeking help concerning my trans status in 2017, HRT February 2018 and fulltime June 2019.

 I will put the number of months with the test results. I think we began HRT at a similar time, for me it wss the start of 2018.

 Imagine living without it. Imagine back on full T with all the extra dysphoria that goes with it !

Notes:
I am taking Cyproterone Acetate tablets and Estradiol Trans-dermal patches.
Cyproterone dose has remained constant throughout
Estradiol Dose started very low and increased every 3-months. I have been at transitioning dose for past 6-months


Interesting result at the 12/13 month mark. My Dr and I believe the issue to be caused by saturation at the patch sites. Most likely due to not putting the new patch further enough away from the previous patch site.

I have NO idea what my E level is right now. I do know that I started my new dosage Sunday last and if my chest pain is any indication (ow ow ow Ow ow OW!!) I can, with zero doubt, say it is having an effect.

Why do good things always seem to have a painful cost? Isn't the pain of GD enough? Or the emotional hurt?

Hi Pamela,

I just got my new numbers in. My labs were drawn on the 5th day after my shot. My new E level actually dropped from 545 (U.S.) down to 493 (U.S.) this time. So my doctor wants me to increase my Estradiol quite a bit, then come back in two months. She wants my level up near 800 (U.S.).

She said since I'm a bigger person I'll have to have more Estradiol to really effect some change. For the record I weighed 285 yesterday when I was in her office. She said my T level was perfect last time (it was 19 [U.S.] I think) so she didn't have it checked this time.

Doe she have experience with trans people?  My fist endo did not, but he did send me to one who had (but was an idiot).  I am now very comfortable with the endos from the Mayo!

Yes she has quite a few trans patients, and they're apparently happy with their results. I've not heard any bad reports about her...but I'm always leery of doctors.  :laugh:

I am sorry but I have found to my surprise that both within UK and within US, E targets for transwomen vary significantly and so all I can advise is to seek your doctor's/endo's specific target.

I have to admit with some shame I did the DIY scheme myself for a short time.  I thought I would see if it was really what I wanted long enough to be able to bail out without too much undoable stuff to deal with.  And I was a tad afraid to being it up with my endo.  She was so great I wish I had forgone the whole search for god knows what your really getting estrogen on the web.  Anyway - just wanted to echo your comments.   As to what is the correct dose and such I think you can get nearly any answer you like depending on how many doctors your willing to see.  Pam probably has a better handle on this but I tried looking at what should be normal (or average) levels of estrogen / testosterone for men and women and its all over the place.  One lab to the next don't seem to be even close to agreement for women as far as normal testosterone levels. 

Anyway - my endo told me it was ok to experiment (Within bounds) the amout of estrogen saying to adjust wait check results then adjust again and when it felt right to come in for labs and see where that was.  In my case it was 400+ something in US units and she suggested I might want to back off a bit.  BTW for anyone curious this was with the transdermal spray type which I wouldn't think could do that well.  *Sure better then injecting every week.  I felt like a tenderized piece of meat.

My endo team just decided that I increase my estrogen injection by 50%, yes, 50%!!!! He wanted m to inject, because their experience seems to indicate (an they are probably the world's largest and best know medical setup) that injected estrogen is the least riskiest delivery form for forming blood clots.  They want me to be somewhere around US 300.  Injecting does not bother me, but I also used to it from my profession (when we learned it, we did self injections).  Any way that is not oral, is a better way!

Yes Jane, a few months ago I was looking into both the expected BT E&T range for ciswomen and the target BT E&T range for transwomen and I found significant differences both laboratories geographically (state to state in US or city/town in UK) and of course between different doctors and endos. Some follow WPATH guidelines more than others but it is not for me to judge.

Hello Laura and welcome to this wonderful website.

I see you are 68 and from Northern California; I am 64 and obviously a Brit. In UK it is customary to have Blood Tests every 3 months for a considerable time as I have done for the last 20 months. I note that in several US states again it is quarterly to start with but after a year or so becomes bi-annually.

I would therefore think you are now due a BT.

             As a hobby been column sorting latest results based on "E2(Int)" using UltraEdit32 . . . seems like AnneK and Taylor Kelly currently comprise almost opposite extremes of the chart average (which would be around "AngelaJade" somewhere in the middle)

At the beginning of this thread there are tables for our BT readings after 3/4 and 6/7 and 9/10 and 12/13 months and therefore you may wish to make your judgement from those. I recall you are now probably 6/7 months HRT? Perhaps if you could try to remember to put the length of time you have been on HRT on your next BT results please. Thanking you.

One thing I have learnt from keeping these results is that results are so erratic both per person (at different times) and between persons (at the same length of time on HRT).

T = 7.2 nmol/l.    Estradiol = 70 pmol/l

May '19
T = 6.0 nmol/l.    Estradiol = 137 pmol/l

July '19
T = 0.5 nmol/l.      Estradiol = 117 pmol/l

September '19
T = 0.3 nmol/l.     Estradiol = 211 pmol/l

I had a bad dysphoria attack after my E fell at my July reading, as it was letting me know it won't tolerate setbacks!

Next reading in 3 weeks.

I am not expecting you to go through all the readings not in tables as that would be so time consuming and besides not all members have listed the number of months on HRT at the time of BT.

However may I just suggest 5 additions if you could please:

Most results here are for periods of at least 3 months and hence I would be most grateful for your 3 month result in September please.

However your T reading 8 (US) 0.3 (Intl) is remarkable and hence it is virtually suppressed. Your E reading of 94 (US) 345 (Intl) also shows a good start.

Ok while my testing is not on the same intervals, here is my 3rd result from this months test.

Before I started

T ng/dL = 743
E pg/mL = 25

2 months with a 2 weeks lapse in the middle

T ng/dL = 8
E pg/mL = 94

4 months

T ng/dL = 15.5
E pg/mL = 134.6

I'm actually surprised and expected my E to be higher and T not to have gone up at all.  I'm on a full transition dosage.

Hello again Danielle

I have researched E Target Ranges for transwomen and was surprised to discover that they differ significantly within UK and within US and within the same State in US and therefore all I can suggest is that you seek the E Target Range as stipulated by your endo/doctor. It may take 9 or12 months to reach a stable E level (in the eyes of your endo/doctor).

Although we are not permitted to specify doses, we are permitted to state that our dosage increased or decreased or remained the same after discussion with endo/doctor. Has yours amended your dosage please?

The T intention is to get it low but not to completely eliminate it as ciswomen also possess it. I consider both your T readings to be good and of course the first is preferable to the second but no problem unless the third at 6 months increases again.

So do we expect E to rise overtime at the same dose?

Mine went actually down.  But the test methods are pretty crude, and you may see a rather large variation between different tests.

I also note your endo desires BT day before injection which is "trough" level which is good advice in my opinion.

E should rise over time (but not necessarily within short periods of 2 or 4 months) and should reach your E target providing dosage correct. It takes time for correct dosage to be determined (several amendments) but the intention is to reach and maintain the E Target over time.

Finally in my target range! Endo admitted he started me off conservatively and only the past 3 months have been effective transition levels.

i think your E is very high, the Mayo folks want me to be around 300, and consider that to be on the high side!  The danger with high E is blood clotting, much more so than overwhelming your receptors!

I must agree that your E level is extremely high and I think your dosage in discussion with your doctor should be reduced.

Without wishing to alarm you, I was given several friendly warnings about my 6 Month E reading in August 2018 of 1662 (Intl) 453 (US) whereas your latest is almost twice that! I recommend urgent appointment/discussion to explain your knowledge of E readings and hence your concern at your own recent one.

I wish you resolution of the matter and peace of mind.

Just as testosterone in transmen reduces estrogen production, the same is true with us but in the opposite direction. Levels around 200-300 PG/ML (734-1,101 pmol/L) are usually needed to reduce T to female range. This will happen quite quickly too, within hours.

Nice steady progression. My T level won't be back until Thursday! I think the high reading from last night was due to the time I took my sublingual tablet and having my blood drawn: 32 minutes apart. The Nov 08 reading was obtained after I drew labs 2 hours post administration, which I think is more accurate given that my Dr. was shooting for 350 pg/ml.

With me they always report < 0.1.  I have never ever seen just a decimal point in a lab report

I'm somewhat concerned. I suppose it's a good thing to see my levels progressively decrease over time, but I had no idea my Dr wanted to knock me flat to zero!

The tests methods are not that accurate to determine absolute zero, that's why they report <0.1, which means as close to zero as they can measure.  I would t believe a report that says 0.0, cause they can't measure that.

I don't know what you take, but for me, without balls, and with hypogonadism, this <0.1 is a very normal measurement.

Linde, exactly! I just messaged my Endo Physician with this reading and I'm interested to see what she says. I thought it would read 0.00 or something else but not  simply  .

I'm somewhat concerned. I suppose it's a good thing to see my levels progressively decrease over time, but I had no idea my Dr wanted to knock me flat to zero!

I got my readings from my circa 5 month endo appointment.  It looks like my testosterone is being repressed, at <7.0 ng/dL, but they didn't give me an exact number.  My estradiol level is at 88.4 pg/mL.  That seems low, but hopefully letting my tablets dissolve under the tongue will increase those numbers a bit.

Thank you for your latest BT results.

I noted that after 3 months you had a very high E reading of 2717 (Intl) 740 (US) and a low T reading of 0.2 (Intl) 6 (US).

Now after 4 months your E reading is reduced significantly to 463 (Intl) 126 (US) with little change to your T reading of 0.2 (Intl) 5 (US). Both T results should please you greatly.

Well I got mine for 6 months now at 3 month my Dr took me if pills and out me on patches because of recently dxed heart issues.  Apparently patches are not my thing because she's going to change them she was talking sub lingual just waiting on a result for potassium retake because my labs there were off anyway.  My 6 mo was 42 t but the patches really messed up my e2 it fell to a mere 49 eeek so looking at a dosage increase and a change wheni get that potassium resultsback

Latest check had my E at 200 and T at 10. Because I do injections these are my numbers at the end of the 10 day cycle. So it's my lowest possible number. I'm just over the year mark btw.

I am truly sorry but I have reconsidered and I must maintain my decision of June 23rd that I shall no longer produce further specific Month Tables but I shall continue to monitor E&T BT results after any period of time. I copy my earlier post below:

"You may recall that in May and August and November of 2018 and February 2019 I compiled tables of E and T Blood Test results after 3/4 months and after 6/7 months and after 9/10 months and after 12/13 months.

Since February 2019 I have been using this thread to incorporate any E and T Blood Test results. Just to let you know that I do not propose to continue producing tables for 15/16 months, 18/19 months etc. If anyone else wishes to produce future tables they are welcome to do so of course and have my blessing!

However anyone may still provide their E and T Blood Test results after ANY PERIOD OF TIME by all means ON THIS THREAD PLEASE if they wish for comparison purposes. I would be very grateful if those that choose to do so would confirm either Intl Units used in UK, Europe, Canada, Australia or US Units please. Obviously I am a Brit but I appreciate most members are my friends from US.

Comparing blood hormone levels with others is not all that effective. You don't know how sensitive others receptors are. Some people have low levels of a specific sex hormone but have high sensivity for it. Others have high levels of a certain sex hormone but have few receptors for that specific sex hormone or have low sensivity for it.
Doses and serum levels are not always all that representative for what results to expect.
I'm on low doses of HRT with major changes even at very low doses.
I am one of the few who don't even need a T-blocker. I'm very sensitive for estrogen, more sensitive than I have ever been for testosterone. During puberty my voice didn't even drop and I have an excellent height for a woman.
Combine that with a bit of hip growth and a bit of breast growth and you have a trans girl who is nearly indistinguishable from a cis girl of the same age.

We currently don't have measurement tools to measure the tissue sensivity to a certain sex hormone. Blood levels don't always mean something.

My Estradiol levels are around 70 pg/ml and my testosterone is around 2 nmol/l but I have had major changes during my transition.
Skin is super soft. Face is smooth and feminine. Body odor is feminine. No need to shave my legs more than once every three weeks.
This has been accomplished with the 3rth lowest dose of patches that are available.
No blocker added.
I must be very sensitive for estrogen.

Just got my 15.5 month testing done, midway through a biweekly dose that is half of what I used to use.  My high numbers have finally come down, and T didn't budge, so I guess that's good news.

E (US) was at 199, T was 13 (US), bottom range of the sensitivity on that test.  Guess I finally got my numbers in the range the endo was looking to hit, out of the 400s. Took a long time, but finally seem to have the "right" levels.   Not sure what difference it has made so far... lot's of hair still coming back on a previously bald head, so pretty excited about that alone.  The rest... pretty meh.

I know we are not supposed to talk about doses but if its ok I wanted to ask if anyone has an opinion on delivery?  My Formularly had me on the transdermal Evamist.  It gave me some good results (I had to dial back)  Thats no longer a choice (Can't even do a PA) so my choices are oral (pills), Valerate Injectables or patches.  I can do self injections (Trained) but don't know if thats one of those roller coaster things where your labs are crazy high, then normal, then sub normal and you go crazy.  My Endo is (I'm sure) ok with whatever.  Oh!  For these "Canadian" pharmacies that require prescriptions and are "vetted" as being legit I could go cash for estrogel (Which I've used in the past.  If anyone has any thoughts on if there is a reason not to go that way I would appreciated hearing it.  Thanks!

Yes indeed your E is exceptionally low and I am surprised. You may consider it to be due to different E Patches or to timing of the BT. If I were you, I would suggest another early BT instead of waiting till April.

I hope you find resolution. As you know several of us in UK including me changed from E Patches to E Gel and I am getting good results from the E Gel.

I had my first blood tests after starting pellet therapy and modifying progesterone intake.

After 6 weeks:

Estradiol is 330 pg/ml
Testosterone is 25
Free Testosterone is 4.3
Progesterone is 7.2

I am no doctor but your E level is extremely high for a "trough" and is rather high for any period. Perhaps you could check that this is in fact your Estradiol (E2) reading please; if not I am sure you could let me have your E2 reading.

The lab paper I was given by the local nurse ( who wasn't supposed to show me) only reads
"Component: ESTRADIOL, Latest Ref Rng & Units: >= pg/ml, 2/5/2020(date): 996

Yes I hope to hear back from doctor. It's been over a week.  She's the head of the clinic and the only prescribing Dr there, and she's only there the first Wednesday afternoon of each month.  So I asked the forum for help.  I'm supposed to inject again  today, and am afraid too.

On the back of these results the endocrine has now doubled my dose. YAY! and I apply it twice a day to maintain the level.
Still having really good results on the gel too. Next results will not be until September now, I am on 6 month checks from here.

Oh, they also switched me from testogel to tostran, as it is a much lower dose and avoids the peaks I was having with testogel (I need to take some T, despite being MtF due to producing none at all). This also seems a much better solution and more in line with cis-female post-menopause HRT in the UK.

My endocrine issues are not normal in the slightest. Having pretty close to 0 T is not the same as having none at all. Most people produce T from other areas of their bodies, adrenals being one such place. What is usually measured is Free T, or the T available in the blood - this does not quite give you the full picture, but is enough for anyone with a functioning endocrine system. I produce no LH, FSH or SBGH hormones naturally (amongst others) so for me there is an actual need to have T in my system and put it there artificially.

That said, I do wonder why some endo's like the T to be as low as possible when transitioning? Most cis-females have a significant level of T in their systems and it doesn't masculate them to any extent. Obviously not as high as cis-males but still a lot higher than a lot of MTF targets I have seen posted on here? A female with low T has a low sex-drive and gains little satisfaction, Is usually depressed, lethargic and feels very weak.

When I apply my T-gel I get a real sense of dread and hate - it seems to me to be counter-intuitive and I feel a repulsion for doing so - But biochemically it is vitally important I do so (and as I said, I am unique in this) but also there are benefits to being female and having correct T levels...

SarahEL your posts make me think hard about counterintuiition with hormones and what's going on with me.
My T is low without any blocker, yet I show just tiny development on E in 7 months.  Believe my T has always been very low, yet I looked very masculine, lots of facial hair etc

I confirm pmol/L and nmol/L are the Intl measurements for E and T respectively. We always use them in UK and hence yours should be measured so.

I confirm pg/ml and ng/dl are the US measurements for E&T respectively.

Yes although I can't see why there's imposition of non-scientific terminology when the science of endocrinology is so important in transitioning. It's even more important when you consider that there are other units of measurement in blood tests. I'd much rather stick to laboratory measurements even when there are multiple units of measurement. For those who don't live in the UK or US it gets even more confusing.

Agreed, Pamela. I've had situations too where reputable endocrinologists have said completely opposite things and on many occasions they, and other medics, have sought a steer from me. I know part of it is about listening to how a patient feels in themselves but the lack of consistent science is one of our problems I feel.

I think part of the problem is that despite an emphasis on evidence based medicine, there is a dearth of evidence on which to base a program of HRT for transmen and women. Couple that with the patient's desire to see rapid results, the informed consent model on which many HRT programs operate for transgender individuals and the use of E2 and P4 to control T and you end up with exactly what you describe. A wide variety of blood hormone levels and recommendations from different doctors.

             I just got my 12 month blood levels from my GP.

Oestradiol 135 pmol/l

Testosterone. 0.4 nmol/l

I am back within the normal male range for Estradiol and to say I am disappointed with my Endo would be a huge understatement.

have my latest bloods, E serum oestradiol 575 pmol/L and T .8 nmol/L  The T is up but this was taken same time as my 12 week anti T inj so that's expected to rise. Recon that was a mis print/ bad test on the E last time .

Perhaps our dosages, managed by knowledgeable physicians, are to change, first to help develop female body features, then to maintain them, and maybe (I hope) to avoid cancers, such as breast cancer.  Mammograms and breast exams can help us in regards to the potential or realized cancer, but cancer still is scary.

While we may all like the idea of having E to change our bodies, maybe as fast as practical, or hopeful!, perhaps dropping the E levels to the least possible later on would be good to avoid some of the cancer risk.

If someone can post or repost or link us to current related medical research literature concerning exogenous estrogen’s risk for cancer for MTFs, that might make for some good reading.  I think this information has been posted before here on various topics, but perhaps newer related research is also worth reading along with the best less current related research.

Blimey. Latest blood results. Eeeek. 4 hours after application of T dose. Fasting test.

Three abnormal results are:

Oestradiol  3117 pmol/L

Sex Hormone-Binding Globulin 29.3 nmol/L

Testosterone 400 nmol/L

The rest normal:

TSH 1.85 mIU/L 1.85
Thyroxine T4 16.7 pmol/L
Prolactin 241 mU/L
Luteinising Hormone LH 6.66 IU/L
Follicle Stimulating Hormone (FSH) 10.2 IU/L
Cortisol 389 nmol/L

Can anyone tell me asap if they think these are cause for concern?  :o

I did these tests privately through a finger prick test, posted off. I'm wondering if this is suspect to be honest.

results this week.
Serum testosterone level < 0.2 nmol/L
Serum oestradiol level 1394 pmol/L

E level is somewhat high probably due to the patches falling off (showers and  during exercise) all the time and having to put new ones on. Im on a different patch to the original make as due to a change during the shortage we had in the UK, and I have asked them to put me back on the originals but they haven't as yet

I am on fortnightly blood tests for a month until my levels come up, and there has been some movement.

so 2 weeks after my last test my E has risen to 248 (intl) and T back down to 0.3 (intl) (this was on patches)

Next week I have another blood test to see what Gel is doing and start a higher dose with pills supplementing the Gel. I'm sort of being a test bed for various methods of HRT!

According to  the Doc at GIC the Jan level was 745 outside of the 600/500 level they want , but the last one I had was 575 but they will not look at that (one before high one was mid 500 /600) the high test was taken day after new patches . They want to reduce my dose , sent them an email but the doc looks like a "I am right " and will not listen.

Titrating very slowly with my estradiol valerate upwards and my tostran downwards. Latest results, taken yesterday:

Serum oestradiol 236 pmol/L

Serum testosterone 5.9 nmol/L

Serum total HCG level <1 iu/L

Everything else is in range.

I would be very grateful for comments. I am guessing you will all think the oestradiol is slightly low? I am a little light-headed sometimes and so I have just written to my endo to increase my estradiol valerate by another notch.

I can conclusively state that the finger prick test is wildly inaccurate as I did one yesterday the same time as the 'proper' one at hospital using a venous draw. The latter are the results I posted a few minutes ago. The finger prick came back insanely out of scale. Clearly useless for anyone taking exogenous or oral hormones. Be warned! xx

My 13 month E level was 420 int'l and T the same at 0.3 Int'l. but I was still having no development, so my Endo doubled my dose! I started to see development within a couple of days!

My GP rang me today to say my 14 month E level had almost tripled to 1200 Int'l and T the same at 0.3, but my GP thinks they don't measure T below that, so it could be lower.

I see my Endo next Tuesday and we will reduce my dose and try to lower my level closer to the 800 int'l target I have set.

My latest results are

I'm alarmed at the E.  My last draw in Feb showed 996 and I have greatly reduced my dose (which was already relatively low) since then.  I'm taking less than half what I was.  Surprised to see that instead of going down, the level more than doubled.  My Dr says she's never seen a level that high but hasn't told me to change, I have an appt next week and will ask again.  Even my last reduction was based on my own judgement.  When I asked about the 996 level i was told to do what I thought best.  My logic was to significantly reduce the dose.  Instead my levels went up.  Hopefully this info helps someone be aware this can happen.

I have also never seen such a high E reading.

You are absolutely correct to point out the possible danger of very high BT results.

Its bad enough that there are so many different ways to take hormones (Injections, Sprays, Patches, Pills, Pills under the Tongue, Compounded paste, Gel Pumps, Gel "Rulers") and figuring out the equivilants? Good luck.

I do wish there was a closer agreement on the goal numbers and more information on Progesterone, DHEA, correct Testosterone levels and? 

Its not just different countries that use different scales but I've seen different "norms" from different labs in the USA so they don't even agree with each other or the labs are done differently.  "Fast Estrogen" versus panels - Ugh!  Plus I'm a bit confused by the levels of different types of estrogen...

I have to credit my endo for dialing back some of my anxiety with her setting a limit and telling me to adjust until it felt right then come back and do labs.  Then adjust down ;-)  I guess I got a bit E2 happy...

Been a while since I have been tested, but at 22 months I just got the following when testing on day 7 of a 7 day injection cycle...

E 254 pg/ml
T 21 ng/dLg

Highest my T has been since all this started, which is a good thing, I think it has been too low previously.  I'm actually at a really low dose now on injectable E, which I guess is good because the bottles are lasting a long time.  As far as I can tell, nothing has changed physically since the first 6 months or so really.  I'm okay with it though, I no longer have any goals to transition, happy enough the way things are now... and much different outlook on what matters to me in the Covid world.

The doc says my E is in the goal range. I'm not thrilled but I understand that due to my age (69), they worry about the possibility of a heart attack or stroke if my E is higher.

Laura, you are on the lower end of the transition range. I don't think it's the E level they worry about, more likely they are worried about raising your dosage due to the slight increase in the risk of clots. If you are active, and there is no history of clots, you may want to think about finding another doctor. Unfortunately, most medical providers worry more about their risks than yours! I just had to fire my Endo as he was trying to treat me as a post menopausal woman! If you are not feeling good about your levels, find another doctor!

I don't know, if that's actually her levels at trough, probably not bad actually, especially since she is achieving low T levels.  I know Kaiser sticks pretty hard as a policy towards the sub 200 E target.  Mine brings it up all the time and that it is an HMO driven policy.  Can't really blame the providers for worrying about their liability.  They're responding rationally to a legal system that punishes them severely for going outside the lines if something goes wrong.

Having said that, every time my endo sees my levels over 200, he mentions that it should be under, but doesn't do anything about it directly other than recommending lower doses of injections potentially. Making the recommendation covers him, I ultimately control the injections though within reason.  I keep mine just over 200, but have been slowly ratcheting down... used to have 400ish levels and it didn't seem to buy me anything extra.  If anything my breasts feel more active since my levels have gotten lower.  It will be two years of HRT soon with what were "good" levels throughout.  You'd be hard pressed to notice that I was anything other than the male sex I was born into and am settling into remaining as on the outside.  I just smell a lot better. :)  Totally worth it!

I assume that these are indeed your trough (or perhaps mid) readings.

As you may recall my E readings have also been high - 1403 (Intl) 382 (US) the last time.

I am planning GRS  and have been advised to reduce my E dosage of E-Gel which applied from August 3rd with the aim of reducing my E reading. We will see  how much my level has reduced when my I have my next BT on October 1st.

My initial test results 7/1/20: 

T = 553, E = 14.5

3 month test results  9/24/20:

T = 80, E = 49.5

The doc increased my E dose by 50% today. I’m on E and Fin.

Finally got my 12 month results - but actually they are 13 month..thanks to the restrictions

So Oestradiol, 337 pmol/L
and Testosterone 1.8 nmol/L

These are international measurements...

endo notes that E2 is too high for post-menopausal levels and T is too low... But I am writing a letter!!

My Prolactin is still high and LH and FSH very low - that is from the tumour..
and then one strange result my ALK PHOS is very low too and no idea why?

I had my latest BTs in September which I have added to the above table. My endo was concerned at my 2 previous high E readings which must be reduced significantly  before GRS next year. Therefore we are both happy I am on the right track with my latest E reading above.

According to  the Doc at GIC the Jan level was 745 outside of the 600/500 level they want , but the last one I had was 575 but they will not look at that (one before high one was mid 500 /600) the high test was taken day after new patches . They want to reduce my dose , sent them an email but the doc looks like a "I am right " and will not listen.

I have also been told that GRS Surgeons in UK require E no higher than 600 (Intl) before agreeing to operate; I note you have been told a strict range of 500-600 (Intl). I always arrange my BT at trough level and although I am reasonably happy with my latest E reading of 804 (Intl), I think is going  to prove difficult for me/you/others to arrive at a point where E is within such close parameters. I assume you are also concerned about their precision and strictness?

Hello again

Aswell as being advised about  my E, I was also advised my T is low enough such that Decapeptyl/Triptorelin Injection no longer necessary and to delete it and remain on Finasteride only and therefore my Injection last June was my last (unless T increases at next BT in December). Were you also advised about your T perhaps please?

Of course irrespective of satisfactory E&T readings now, I was also told as I am sure you have that, surgeon having agreed to surgery - as you know I am virtually decided on MDV - then we have the problems of cessation of hormones for several weeks before surgery. I am concerned about managing on no hormones!

This excerpt from the University of California, San Francisco  "There is no evidence to suggest that transgender women who lack specific risk factors (smoking, personal or family history, excessive doses or use of synthetic estrogens) must cease estrogen therapy before and after surgical procedures, in particular with appropriate use of prophylaxis and an informed consent discussion of the pros and cons of discontinuing hormone therapy during this time. Possible alternatives include using a lower dose of estrogen, and/or changing to a transdermal route if not already in use.[62]"

So I avoided this topic like a plague for a year now. I just had my first appointment in over a year. My previous one before that was November last year, & my results were devastating. My E was 133 & my T was 355. My previous results before that were E 130, T 120. I was supposed to have a follow up appointment in March, but that didn't happen, for obvious reasons.

I was finally able to get back in to see them last week, & dreaded the numbers. Just got them back:

E:361
T:43

Needless to say, that made me do a happy dance. Massive improvement, to say the least.  :icon_dance:
