First off, having these feelings doesn't make you a freak. It makes you an introspective human being trying to get to know yourself.
My advice would be to find a therapist, preferably one with experience or a special interest in gender issues, and talk to him/her IN DEPTH about your life and your feelings. Don't hold back. Tell them what you are thinking, feeling, what you want need and fear.
None of us here can say "yes you are trans, here's what to do." But we can offer support, compassion, and understanding.
