It depends on your state. But considering you just came out, and I'm assuming you haven't been to a medical professional, I'd safely say no for the gender marker. Name change can occur anytime, you just need to talk to your parents. It may take a few months for it to go through, but it is relatively straight forward.

For now, find a good gender therapist. Try contacting your local LGBTQ support center, if you have one, and they might know. Skype is also an option, and my therapist is amazing. He isn't a gatekeeper, he is only there to help you successfully transition. http://therapists.psychologytoday.com/rms/name/David_Baker-Hargrove_PhD,LMHC,DAPA,CCFC_Orlando_Florida_45705

So, you have the support of family, now do something. Get your letter for HRT, then find an Dr who will prescribe HRT. That might somewhat trick at your age because some people see it as controversial, but you should be able to get on testosterone blockers at the very least. And trust me, that will be more precious than gold to you at this time. Above all else, be proud of who you are, and that you are leading life by your terms.

In the US, much is determined by the states, not federal. This includes laws regarding name changes and gender changes. Name change is usually pretty easy but can only be legally done by parents if you are under 18. Hormones are very iffy under 18 but hormone blockers are possible. Therapist may or not be possible depending on your parents. Often 3 months is enough for a therapist to decide if HRT is the next step for you. Any medical decision before 18 will take your parents consent unless it regards pregnancy/abortion.

Good luck.

Once I changed my name legally in the spring of senior year (which you can do at any time, and is irrespective of gender markers), I contacted the school and asked them to change it for my diploma, transcript, and records.  They did so.  In addition, depending on how in with the administration you are and what the school's actual policy is, you can try to request that they change gendered correspondence and situations to reflect your identified sex.  It wasn't easy, but I was able to do so and as such could wear a dress for graduation.

Also, I started spironolactone at age 17 and three months and started estradiol at age 17 and seven months.  This was, of course, with parental consent, which you'll need until you turn 18.

it depends solely on the state in which you live.  being a minor, you may not be able to legally change/correct your legal documents until your 18.  If you wish to legally change your name, you must petition the local courts for a legal change of name.
If you wish to legally change/correct your sex on your documents, the procedure varies by state.  some states only require a diagnosis of GID/gender dysphoria.  some states, you cannot change your birth certificate, other states require a surgeons declaration letter of "irreversible genital surgery" or to that nature.
