UK/ NHS: What exactly happens at a gender identity clinic?

Okay so I know this seems like a stupid question, but I'm really anxious about talking to institutions about transitioning, and it would help if anyone could tell me what kind of questions they were asked, what info they were given, how many sessions, and if they're all solo or if partners etc are allowed to be there.

I was also wondering how much information GPs want before they'll refer you. Will it be as simple as asking 'I would like a referral to a gender identity clinic' or will they ask for my life story?

I'm sorry if this info's already out there. I know about waiting times etc, but nothing else and the uncertainty is stressing me out. (I'm that annoying kind of person on road trips who needs an exact itinerary) I'm just working myself up trying to think of how to come up with evidence of being out and convenient childhood memories and if it's not necessary I'd sleep easier.
