Boston Health Care for the Homeless program cares for transgender patients in specialized clinic.

In most ways, Helen's appointment with nurse Pam Klein resembles any other check-in with a healthcare provider. Klein takes her vital signs and weighs her; asks her about her smoking; screens her for depression and examines her tongue, which has been bothering her for weeks. Then Klein looks at the electronic medical record (EMR) on her monitor and grins: "The system says you need a pap smear," says Klein. "But we know you don't."
