Reasons given for name change.

I live in Quebec, and currently filling out the application for a name change. The requirements for name change:

-a letter from a psychiatrist or psychologist attesting to their transsexuality (though this is the specific requirement according to Department of Civil Status, some have succeeded by using a letter from a family doctor or a sexologist instead);

-some proof of physical change towards the gender to which they are transitioning – this could include proof of hormone therapy or surgery, though in some cases a photograph showing gender presentation has been accepted; and =

-proof that they have used their chosen name for at least a year. This proof could include letters from an employer, school, community worker, doctor, family member, or friend.  It could also include bills, receipts, a lease or other documents in their chosen name.

On the application, under the section where they ask for you to state the reason for the name change I wrote:

I am a transgender individual, female to male. I am requesting a name change due to the fact that my birth name does not reflect who I am. Soon I will be starting hormones, but I find that it would be problematic to start hormones and still have my birth name on my identification papers. Walking around with facial hair, and other male characteristics which hormones give you and still have my name being Jolyane would be extremely confusing for people and maybe even dangerous to my life. As a safety measure I would like to request a name change right before starting hormones. If this passes, a letter of my therapist will be sent, and letter from my physician. I will also add a letter from my parents/relatives and friends attesting that I have been presenting male for many years now, since the age of 15. I will also add a picture of myself to prove that I am presenting as male.

Sounds good, Jayr.

Man, I didn't know Canada required this though. I didn't have to state any reasons for changing my name and didn't even have to disclose the extent of my medical history while doing it. In my opinion, this is such an unnecessary hassle and must really suck.

I agree with you. Quebec is the worst province, they are extremely strict.
I didn't even tell you that it took around 6 months to process and around 400$.

And if I want the F changed to M, I NEEd to be taking hormones and undergo a hysterectomy.

I have my American papers though. I simply do not live the the US and therefor I doubt I can ask for a name change for my American documents.
So complicated.
