Having trouble choosing an online therapist.

I'm currently searching for an online therapist because those knowledgeable in gender identity are lacking in my area. Seems to be a common complaint, huh?
The two most important aspects, besides the basics, is a sliding fee schedule because of my financial situation, and sessions through skype.
I'm considering Marybeth Markham, Dr. David J. MacIntyre, or Dr. Lori A. Jespersen.
Has anyone used these therapist?
Markham is a Mental Health Therapist, would she still be able to write a hrt letter? Would she get a Dr., or similar, title or no?
Dr. MacIntyre's website doesn't even say anything about gender.
Can't find much info about Dr. Jespersen.
I've been fretting about this for days, I just want to start moving forward in my life but I don't know what I'm doing, even when it comes to just the idea of sending that initial message to a therapist I don't know how to word it.
