I look for ob/gun doctors in Jacksonville. Gainesville or in needed Orlando Florida. They say my letter from Thai surgeon is no good enough. They say at DMV and birth certificate place I need doctor with DEA number to write letter.

Too true, but the same happened to me in SA... after all none of the administrating staff will be allowed to verify that vj-fact. I went to our local LGBT clinic and got a letter stating the required and it cost me nothing.
So long it is a locally registered doctor - as seems to be the requirement in Tristan's case too, it ought to be OK...
