I second Aussie Jay on this: join the Yahoo Phalloplasty group if you haven't already--should be lots of resources there. I don't think there are many guys here who have had bottom surgery and (to my knowledge) I haven't spoken with anyone here who has a phallo.
I went to Dr. Miroslav Djordjevic in Belgrade, Serbia for metoidiplasty, but he also does phalloplasty. NSFW, surgeon's site with operating room phalloplasty photos:
http://www.genitalsurgerybelgrade.com/phalloplasty. I know that he implants erectile devices and he is a urological surgeon, so the urethral lengthening should be possible as well. With my surgery, he created a scrotum and inserted testicular implants and I also had a vaginectomy. I think it was the gynecological team that he works with that did the vaginectomy (technically a colpocleisis, which from what I understand is a procedure that has a low incidence of blood loss requiring transfusion).
As far as my experience with the surgeons and nurses, they are wonderful people; really can't say enough good things about their kindness and hospitality. The surgeons speak excellent English (the nurses a little bit), so there would be a way to communicate with them. Technically, I think the surgical team is very skilled at metoidioplasty, but cannot comment on phalloplasty because I have not researched it. Sorry I can't offer much more than that, but congrats on having the phallo done--it seems like that is in many ways the hard part. Going into surgery and worrying if the phallus will survive afterwards or not would make me very nervous.
