Requirements for name change in PA

You have to publish a notice in the legal journal and either the post gazette or tribune review

Another update

You don't need any forms of ID for name change.

The only things you need to file in PA are:

Fingerprint Card
1 - 8 1/2 x 11 envelope addressed to state police
2- letter size envelope. One addressed to court house and the other to yourself.
Name change petition.

They send your fingerprints to state police. It takes about 4 weeks.

Once they receive it. They will send a letter in the mail. You will come back and schedule a date for the hearing. You will also advertise in legal journal and post gazette/trib.

Than you will have your court hearing. and name change is done!!!
