Therapy Letter?

I have obtained a letter for HRT in the past and unfortunately ran back into the closet.  In that process I burned the letter and have not really spoken with the therapist since other than to cancel my meetings with her.  How do I handle this going forward?  I would assume that she may require that I continue to speak with her before she issues me the letter again.  Anyone ever been through this?

I had not seen my therapist in a while, but when I asked him for a surgery letter we just had to have a catch-up meeting and he wrote it.  He also referred me to a psychiatrist for my second letter.

I would just contact her and see.
