medical reasons to get top surgery

I'm going to try and give my doctor medical reasons for why I need top surgery so that my insurance might cover it. (I don't expect it to. Just wanna try.)  Can someone give me suggestions?

The only reason would be a breast reduction and even then it's not too likely the doc would take your chest all the way down to flatness.  Most are obligated to report how much tissue they removed, your size pre-op, etc.  If it seems like they removed too much insurance might not pay for it, and they might be investigated for insurance fraud.

If you get a mamogram done and you have lumps in your breast {cancerous or not} I have heard that you have the option of a complete removal.
