I happened to accidently stumble upon the Mass information, but since I live in RI, I had to do a bit of hunting to come up with this site:

Here's the name and gender change kits for RI, apparantly it can vary a little from town to town (*sigh* RI just has to make everything difficult  :P )

I just wanted to add, that the link says you'll need to appear before a probate judge. For anyone who's not familiar with probate courts, they're usually just held at the local town/city hall, usually only once a month unless it's a bigger city/town. Most of the probate judges are really nice, and very casual. The small town probate judges don't even wear robes (except for the guy in Scituate), so you don't need to get all worried about what you should wear before you appear in front of them.
