Muffinpants : Obviously I don't know your endo, but I went with my wife to the first 2 appointments because I'm better at remembering detailed stuff and she was too excited to pay attention.

Nobody batted an eye. (I'm going to go with her to her first gyn appointment soon, too, and the nurse on the phone actually *encouraged* me to!) Her endo did a fairly quick intake exam of the sort mentioned here - does she know why she's there and the side effects/intended effects of HRT - and then ordered bloodwork. As soon as that came back and she'd interpreted the results, she sent a prescription to the pharmacy. So it wasn't exactly at the first appointment, but there was no "second" appointment needed to get the prescription, if that makes sense.
(The 2nd appointment was in three months; the endo's policy is a recheck every three months until a year has gone by with normal bloodwork every time, then every six months ditto, then yearly. Although I think it changes back to the every 3 months for a year schedule post-op just to be on the safe side.)
