Job Application Asking for Birth Name After Name Change

I am currently applying for a part time job at a local pet store and the application is asking if I have ever been known by any other name, including if I have ever changed my name.  I am extremely hesitant to provide my birth-name though marking no and later finding out that I did change my name could be punishable if I were to be hired.  It is an online application so I am forced to mark either yes or no and provide that information to submit it.
Has anyone have had any experience with this?

Yes, on most of the applications I fill out it asks if I ever gone by another name. I mark yes, and put down my old name. It is grounds for dismissal at most places if you lie. Plus, they are bound to find out anyway. Personally, I don't think it is a big deal. Then again, I am an out and proud type trans girl.

no, but since I have a female ID I always registered female,
also if I tell them I have experience ex working in a store and they want to see some provements, or school grades I have to explain to them that those are in another name cause I have changed it..

I figured and provided that information.  It is so incredibly frustrating though; I hoping to receive another job that I have applied to than to get this one, so it is not a big deal in the long run.

Professional jobs don't ask this. Just retail jobs seem to. Hopefully you can not have to deal with this for much longer.

I have to disagree with this. Most of the jobs I apply for have a very strict security check, therefore they need to know all about your past.

I think it depends on how you hear of the opening and the field.... like with my job I got it through networking so I didn't even have to fill out an application, it was just like "here's my resume" sort of deal. Though definitely jobs in banking, accounting, etc. will do a through background and credit check and they'll want any previous names.
