How to word your letter?

Ok, my hysto surgeon is willing to help me out a bit by writing one of the letters needed to get my gender marker changed in my State. Here in NC it takes two letters from two surgeons (that also have to be notarized) to accomplish it and bottom surgery is a long ways off. The gyno I have seen since 2008 was also my hysto surgeon and she wants to help me out but we're kinda stuck.

What is a way to word the letter where it would be good enough to use to get my marker changed? She doesn't really feel comfortable saying she performed a gender reassignment surgery on me because she is afraid it will come back and bite her somehow. I don't blame her for that at all. We just don't know how to put it where it would slide by in court and I have never been able to talk to anyone who has had this issue.

Should she say she surgically castrated me? I just don't know how to get it worded. Anyone have any experience with this? I think the legal mumbo jumbo is the most p.i.t.a. part of transition.  ???

Thank you Devlyn, those links actually helped a lot.

"Irreversible" does seem the way to go.

Just hope I can get this done asap since I'm trying to get a part time job. It's sad that most people get nervous about the interview process or if they'll get the position. I on the other hand always worry about that moment where they need to see and copy your SS card and license. My name has been changed forever but that big "F" haunts me.
