I'm a pre-op FTM so this list might be geared more towards pre-op FTMs, but I feel that things like the TG therapists would be useful for MTFs too. Feel free to add to this list!

Clarke Institute Gender Identity Clinic below was originally not recommended. Reparative therapy was used and HBIGDA Standards of Care were not followed,
but the Clarke Institute has been renamed CAMH, and while it still does reparative therapy on trans youth, it is
the only eligible body to refer for public-funded surgery in Ontario. It now follows the 2001 WPATH standards for adults. (Sarah7)

VCH in BC offers free transgender services to those over 19. "You would be assigned a general practitioner who has specialized part of their practice in transgender health care, and they would be able to assist you through the whole process. This often includes the hormone readiness assessment." according to my correspondent from BC transgender health program.
