I recently passed the first month mark on HRT (yaay!) and thought I would make a post about what it's done for me so far, since there really aren't many out there for stage in transition.
Anyway, my information is: 18 years old, 6'0, 135 lbs, on Estrofem (taken sublingually) and Spironolactone
The first thing I noticed when I took my very first dosage was a huge endorphin release. I remember being so mind numbingly excited that I was literally jumping up and down for joy, lolz. This was, of course, psychological and not the result of the hormones but of being so thrilled to finally be on this path

2 weeks passed with ups and downs in my mood with no noticable effects that can't now be chalked off to the placebo effect. It was around the second week mark that I noticed my nipples began to get more sensitive. Not sore, not painful, not even tender really, but more sensitive.
Over the course of the weeks this sensitivity grew, to the point where it is now very noticable. There is still no pain but there is some soreness if I apply pressure (if I don't touch them I don't notice). But they now have very noticable lumps underneath them, prolly just under the size of a gulfball. They have also visibly changed appearance. I don't know how to describe it but to say they now look rounder, mebbe darker, larger and more prominent, etc. They seem to be starting to protrude the smallest bit and take on a cone shape appearance. Also, they do produce tiny beads of colostrum when pressure is applied!
I have also noticed that I have been experiencing severe mood swings. Most of the time I'm more chipper, happy, upbeat, and self-expressive than I have EVER been in my whole life, and sometimes I get really, really happy for little apparent reason (mebbe the knowledge of how amazing what I am doing is!). Then there are others where I get tired and irritable at every little thing and spaz for no reason. And others when I get super depressed and dysphoric and don't even want to get out of bed. The trials of womanhood!
Also, I have had a large increase in my appetite. I noticed this pretty early on and it has increased even more in recent weeks. I ate over 3,400 calories the other day and still felt hungry when I went to bed, and felt like I was starving to death the next morning. If I don't even for more than a few hours I will be hungry, no matter how heavy my last meal was. This is helping me gain a good amount of weight though, which is good, since I have been recovering from mild anorexia for some months now (my BMI is still low, but not nearly what it was at my all time low). Plus, it's good to give the hormones some fat to work with!
In addition, I seem to get tired out of the blue. I almost never took naps pre-HRT. Now I find myself napping for hours a day and sleeping at least 10 hours a night. Sometimes as high as 12! I was not expecting this one, and it has been sort of a pain.
Lastly, I would say that I have noticed my features DO seem to have gotten somewhat softer. Not much, not much but there is a definite difference between pre-HRT photos and photos I took as recently as today. I now look back at some of my most feminine looking photos from before I started HRT, and I am still more masculine than my most masculine recent photos. Others have even pointed out a difference in my appearance including those who don't know I'm on HRT. Just the other day my mother said I looked different somehow. She said "Your cheeks look fuller, and right here *running finger along her jaw line*". Unfortunately, this has only made me look like androgynous/like a 12 year old boy at this point XD. The other day a woman commented that I looked MUCH younger than 18. Hopefully the hormones will keep feminizing my face!
Oh, as for body hair, really can't say. I honestly haven't noticed much reduction. I don't have all that much to begin with, and what I do have is mostly thin and fine already. My legs still get prickly pretty fast after I shave them, which is a bummer. Hopefully that will start picking up soon.
No sign of body fat redistribution into a more feminine pattern. My facial complexion (which is so-so, not really bad but not all that good) does seem to be improving. Hopefully I get the porcelain clear results others have said they experienced, but we will see!
Otherwise, that's about it. Obviously, I am still very early on, so things will take time. I'm really not looking for much until around month 3 or 4, or even 6 months. Would you say my results are typical for this stage of HRT? What did you experience 1 month in? More, less?
