Top surgery and weight

There are a few surgeons in my area that have provided very nice ftm top surgery results.  However, all of the *nice* results are on small(er) guys.  The larger guys all ended up with the half side boob effect, which looks pretty strange in my opinion.  (It looks like someone poured some skin onto a shelf & then removed the shelf, while the skin retained that shape.)

I've been working toward losing weight, through diet change & increased physical activity.  However, the size of my chest causes severe discomfort when it comes to movement.  The amount of binding required to satisfy my comfort level isn't healthy, & therefore, I don't do it.  I do bind, mind you, just not to that extent. 

Currently I'm a 42c in bra sizes.  I've lost 35 pounds thus far & have lost some weight in my gut.  My chest is shrinking faster than my gut & my binder is now a little too big in the chest but still the right size in my abdominal area.  With the size of my gut, I'm reluctant to go for a short binder.  Should I get a short, smaller binder for my chest & then wear the longer one over it?

In a way, I feel stuck in a catch 22.  To get where I want to be with my activity level, I need a male chest.  To get the male chest I *want*, I need to lose the fat first.

Any suggestions?  At this point, top surgery is a much larger priority for *me* than HRT.

My surgeon told me to be at the weight I want to be at. She also said that the side boob effect is caused by having a lot of fat under the arms. Not really chest size. I'm a 40 DDD/F, but I don't have underarm fat so she said my likelyhood of having dog ears is slim.

I sometimes wear the shorter binder and a longer one over it.
