Not really sure but... Could a MtF and a FtM switch parts in theory? I mean if it was a accepted into the body like any other organ transplant could it work?

so, the way to avoid rejection is through lifelong immunosuppressant therapy, which is kinda brutal. given already extant surgical options, nobody's really clamouring for that.

i mean, in theory? yeah, sure, of course, if you imagine 100% preservation of tissue, which is in practical terms impossible. but there are still questions about how you could possibly carry it out that would require growing cultured tissue via stem cells and by that point you're better off waiting for the whole package to be delivered through stem cell therapy. like, transplanting a penis is all well and good, but where are you going to put it? how are you going to attach it? your best bet would be a cadaver, so you could properly make a hash of the surrounding tissues to make sure you get all the parts you need. keep in mind that the high likelihood of rejection means you would need to rip out the entire set of plumbing, which would mean you would literally be left without sufficient tissue to create even the most rudimentary urinary shunt. in short, it would be gross.

same goes for the ladyparts. you could argue that this setup is more feasible, but nobody's even successfully performed a uterine transplant from a cis lady to a cis lady. i suspect we'll be able to do this one in a not-too-far future, but stem cell advancements may obviate the need for transplant in the first place, which would make it more of a cultivated allograft. medicine is exciting, just not yet, or something.
