Where can I find a sample HRT letter? / Endos in Maine?

My new therapist said she doesn't have a problem signing an HRT recommendation letter, but she wants me to do the work and basically (as I interpret her words) provide her with a letter which she will sign as long as she doesn't disagree with it.  If there a 'form letter' out there or any samples where you can just fill in the name?  She prefers not to go into details that came out in therapy, just give a thumbs up.  Again, this is all my interpretation based on talking to her for one therapy session, but I think that's right.

Also, while I'm asking that I'll go ahead and ask, does anyone know of any good endocrinologists in Maine? North and east of Augusta if possible (Bangor, Belfast, Ellsworth, etc.), but really all of Maine.

If I was home I could copy/paste what mine said, which was basically verbatum what my Endo wanted to see in a letter. From what I recall basically they're looking for these things.

1. You have been diagnosed with GID.
2. Other mental issues are under control (that is, you're not in danger of hurting yourself or others.)
3. You have the ability to take the medication as prescribed.
4. You have the ability to understand the risks involved.

I'm paraphrasing here, but that's basically what my Endo was looking for.
