Ask the clerk what that means and also ask when name changes are heard and what judge it is. Some matters are only heard on certain days.
I just did an expungement for someone, and in the date section I had to get the dates expungements were heard in thst court, choose when I wanted to be heard, put that on the form, and send it to the clerk. The clerk received it via mail, put it on the docket, and sent out s notice to the petitioner. This may be similar for the name change.
