Name change in Massachusetts, Stupid question

So today I recieved a form from the court to fill out so they can finish processing my name change. 
There is a Date Requested, and I am unsure exactly what they are looking for.
Is it the date that I want my new name to take affect? Is it the date that I first filed the paper work?

I believe it is the date you are making the request although it may mean the court date. Call the clerk of courts and ask as Papers in error can slow things down.

I am not 100% sure since I haven't looked art the paper you have, but sometimes that's the date you want to be heard. Ask the clerk what that means and also ask when name changes are heard and what judge it is. Some matters are only heard on certain days.

I just did an expungement for someone, and in the date section I had to get the dates expungements were heard in thst court, choose when I wanted to be heard, put that on the form, and send it to the clerk. The clerk received it via mail, put it on the docket, and sent out s notice to the petitioner. This may be similar for the name change.

edit toadd: may I ask what court you are filing in? I just started doing some volunteer work over at Middlesex P & F, that's why I ask.

I asked someone who is familiar with court proceedings and she just told me to put todays date. So thats what I put. It will be mailed back to the court tomorrow.
I filed in Plymouth County
