I don't know how close you are to your doctor, but maybe you could try to think of him as your friend.  I totally sympathize though, I hate talking to doctors about anything intimate....
Discord, I'm howlin' at the moon
And sleepin' in the middle of a summer afternoon
Discord, whatever did we do
To make you take our world away?

Discord, are we your prey alone,
Or are we just a stepping stone for taking back the throne?
Discord, we won't take it anymore
So take your tyranny away!

think of your doctor as a complete stranger. someone who can't react negatively even if they want to. someone who has to be supportive in order to do their job.

write a note to take with you "i'm transsexual. help!". or something very short. give it to your doctor if you can't make yourself say it. your doctor should ask you the necessary questions if you do that.

these would be things that i did myself. i managed to say it, but i did bring a note as well. doctor was really helpful, though it didn't get me far because of stupid rigidity of the system here. and laws that exclude me.

What sort of doctor?  A general practitioner?  Let me say, there is no reason to be afraid.  Doctors have seen it all.  Seriously, ask anyone in the medical field what they deal with on a daily basis.  Most of my family works in the field, and you would be surprised at some of the things they see.  Having a transgender patient come out is not a big deal at all.  Sure, they may not expect it, but it isn't the oddest thing in the book.  Don't worry so much.

Take a note with you or email the doctor the letter and show up. I'm basically doing the reverse from you as I haven't told anyone in my family yet. My therapist that I'll see in two days will be the first. You gotta make the first step in your journey, no matter how difficult it may be.

@ ukftminneed, If you were relaxed about coming out to the people you mentioned, this is no different, the medical profession is there to help in whatever issues you are experiencing. You believe you are transgender, well let your gp know when you first sensed this & whatever your learning experience is, if you cross dressed & when, tell your story (we all have one). This may not be your Doctors specialty & will probably refer you to a gender Doctor or a gender therapist. Your gp will send you to a lab for blood tests. I also had an abdominal ultra sound. Just be yourself & go with the flow.

Good luck.

This is exactly why you need to see the therapist! You still have issues that need to be resolved before you can move on and they WILL be able to help you. Coming to terms with who you really are is very difficult at times. Grief comes in my forms and is hard to cope with when you have nobody to turn to that has real experience in dealing with it. More often than not we are beyond hard on ourselves to deny our true feelings. I know I am and it took me a while before I came to terms with who I was and what I had to do to fix my problems. Take that first step, it may feel like it's as tall as the tallest building you've ever seen but you'll thank yourself later after you start the path to your real self :D

Blood tests are probably just like when the Doctor has them drawn at your annual physical. It is a measure of how healthy you are. Not to worry. Just BREATHE.
