Revision questions?

I had peri like 2.5 years ago with dr fisher. I anticipated needing a revision since i was borderline for getting peri. My chest is undoubtably male and really doesnt look that bad except it looks like i've lost a lot of weight. I'm actually losing weight now which is making that even worse. A revision still probably isnt in my near future, maybe a few years, it's pretty low on my priorities. I've just decided to do some research ahead of time. I'm thinking I want to go with Dr Medalie, or maybe with Dr crane and have him do it the same time as i get my implants put in. Dr fisher was great but medalie was always my first choice (but i couldnt have surgery with him since he didnt operate on minors). So here are my questions.
1. Is there a way in a revision to move my nipples up a bit? They're kind of low and when i lift my arms they cave in under my pecs a little.
2. Would removal of the excess skin require making another incision besides around the areola? Or would it work just the same as the original procedure. I'd rather not have an extra scar.
3. Do you have to be under general anesthesia for any type of lipo? Because it seems like the rest of it could be done with just a local anesthetic but I'd need some lipo under my armpits.
4. Is there any way I could possibly NOT need a revision? Like i know you cant say for sure since i'd rather not post a picture on here but i guess im basically asking if any of you guys needed a revison and found a way around it. Either with working out or something else.

Only a surgeon is qualified to accurately answer your questions.  But I'll give you my two cents:

Yes, but I'm sure to relocate the nipples there would need to be a nipple graft.

It depends on how much skin there is.  Sometimes they can make fresh incisions over old scars to minimise any new scarring.  But if you have large amounts of skin, it's possible they would need to make a new incision.

I'm pretty sure that I've seen video of people having lipo whilst conscious (obviously having an epidural, etc. for anaesthesia).  Not sure, but they'd probably do this during the revision - and if they are cutting into your skin, you're likely to be under general anaesthesia.

You need to see a doctor and ask them these questions.  No one here is really qualified to answer.  We can't see the amount of skin you're talking about or any of the other issues, so it's pretty pointless to say "yes" or "no".  If you have excessive amounts of skin from weight loss then no amount of working out will tighten it to your expectations.
