It's when one person says something to someone who then tells someone else, usually with a tiny little piece of the original information altered in some way (usually due to the intermediary mishearing or misremembering it); it then gets passed on and on, changing a little bit each time until it finally resembles nothing like the original comment.
(So as far as I know it has nothing to do with voice surgery... but don't quote me on that!!)
