Florida name change documentation: addresses

I live in Florida and want to change my name, but the form wants every address I've lived at since birth.  Um.  I don't know most of them...  Can I just put down cities and dates for some of them?

I live in FL, and am dealing with the same thing. I think if you just try your best, it should be okay. A lot of things ask for info 'to the best of your knowledge' so, as long as you aren't hiding anything and legitimately don't know, I doubt there will be an issue. Or, at least I hope there won't be an issue lol.
