Maryland gender marker change questions

I almost have everything I need to submit to the medical board to have my little M changed into an F.  One of the things that has been requested is a letter from myself, though.  I know the other letters need to follow a very specific format, thank goodness I've been dealing with a doctor and therapist who both specialize in trans* people.  Does mine as well?  I can't really think of any way to write it that doesn't sound stupid and come to about 3-5 sentences....

Here's the letter I wrote to the MVA (I put the underlined text in box and outlined it in yellow so it would not be missed). Hope this helps.
