If there is a College or University close see if they have a Psychology Department. I found my Therapist at Oklahoma State University and with the scale they use it is only $10.00 a session. You do not even have to be a student and they can issue letters for HRT and SRS.  :)

[EDIT] Crap... noticed right after I posted this that you said NC, not DC. My bad.

I know from a relative that JHU up in Baltimore has student-provided dental services, so it's entirely possible that they have psych services that can do what you need and maybe even a sub-dept specializing in trans needs. This was actually helpful for me, too, because I'm looking into the same and have budget concerns, but probably wouldn't have thought of that if not for Jessica's reply. At any rate, I'll keep you updated if I find anything useful in my own search.

P.S. Another thing to check (which I haven't yet but will soon) is if there's a thread of gender therapist and other trans-friendly doc recommendations, possibly by state even, that already exists somewhere on this site.
