Coming to the US on H1B

Am coming to the us on a work permit. But now my passport pic is you know my before... And hey we obviously are different people.. Any suggestions..

There is no way I can get my passport updated for a new pic and leave alone get my gender marker changed

First each country has passport regulations as to how they identiy gender. Some will provide passports without SRS others will allow with your doctors letters. If your country allows this then get it done asap.
If you opt to go with the original passport then the picture must look like you, be it male or female. If not you will encounter delay as they throughly check out your ID. The US has laws that protect our rights but you also have to assit in making sure they can ID you. Best your ID also has fingerprints, and I'm sure you had your fingerprints taken. I always advise TG to ensure their ID include their fingerprint and also more hi tech - your DNA profile.
I fly around Asia ala male mode. I have long hair which is in a pony tail when I travel and after 18 months on HRT my facial feature have become softer BUT they can recognize me after a few deliberate looks.
When we first start our journey the perception and moral values all dramatically change in wonderment. As we evolve further it all becomes normal again but the journey has changed us forever.

First of all, congrats on the H1!

I look different from a number of ID cards in my wallet (DL, concealed carry permit, work ID). Whoever needs to see them look at me, they see it's me and there are no issues. My passports (US and foreign) also look different. In one of them I look like Paul Bunyan and some of them I have what can be called a "baby face" and in one of them I look like an older woman. When taking ID photos I really didn't care much about my appearance, and work has a good tendancy to screw up photos with dark lighting. Others are portraits I took in our home studio.

I believe the visa itself will have a photo in it, but it may be b/w and not color.

The TSA has guidelines on identification for transgendered/transsexual people.
