Okay so I've decided to start looking into Top Surgery. I'm 34 and live just outside of Chicago. I am not on T and have not been talking to a gender therapist. Can anybody tell me where and how to start looking for a surgeon, what requirements they ask for, and approximately how much the surgery costs, before insurance?
Thanks in advance.

I can't answer a lot of your questions, but I'm also curious. I won't pass at all without top surgery. It's just not going to happen. But I am meeting with a therapist, and have a t appointment next month.

I know that everything I have found online puts top surgery around 8,000 dollars.
Because being awesome just wasn't enough.

Figured it out the first time: 1994
Figured it out again: 2002

There are two excellent surgeons in the Chicago area (Dr Loren Schechter and Dr Steinwald).

I don't know the particular requirements of the above surgeons. Almost no surgeons require T anymore. Some do not require you to talk with a gender therapist. I think it's a good thing to do if you are able to find someone competent.

Not all doctors take insurance (very sure that Dr Schechter does, otoh, insurance tends to require therapy). The reason they will require therapy is that's the way they figure "medical need".
You should expect an automatic refusal which you should fight.

Bear,

I live in Chicago and decided to travel to get surgery with someone whose results I really liked. I think it's a matter of preference. I have only recently seen some of Shecter's results, and I had a friend who had to have a revision with Steinwald and said he would've gone to someone else if he had to do it again. I have seen some good results from him since then, but I also heard that he is moving to Colorado and not taking any more patients in his Lake Forest, IL practice. That would be worth checking. If keeping your nerve attached to your nipple matters to you, though, I think Steinwald does some of the best "t anchor" or "pedicle technique".  There is also a plastic surgeon who kinda flies under the radar in Chicago, Dr. Horn.  His site may not even show the top surgeries he does, but you can look at pictures in his office.  If I couldn't swing the flight/hotel, etc, to Garramone in FL, I probably would've driven to Medalie in OH. Some things I have heard make it sound like he doesn't require you to stay local too long.  Also, if my chest was small enough to go to Medalie for keyhole, he would've been my first choice.
There are lots of surgeons, happy hunting and congrats on taking the step.

Also, Bear:

You could consider going to an in-person discussion group.  I know Man Cave meets at the Center on Halsted on Friday nights in Chicago. Not everyone has had or wants surgery, but I have found local transguyz to be quite generous about-dare I say proud of? :)-- showing you their postop chests. It can help to see results in the flesh. A couple years ago, I went toa top surgery topic night at Genderwork (not sure if it's still meeting) where 5-6 guys who went to diff surgeons showedtheir chests.

Heck yeah! After I get it done, I'm gonna take my shirt off like, constantly, lol.
Thanks everyone for responding, looks like I have a lot of research to do and appointments to make. I really need to "hit it off" with Doctors. Anyone who won't take the time to listen is out, anyone who can't take the time to answer questions is out. Which is part of the reason I didn't follow up on getting on T. The guy over at HBHC barely looked me in the eye, couldn't answer a lot of my questions, and was in a hurry to get to his next appointment. I'm still not sure if that's a step I need to take. I seem to change my mind every few hours...

Good for you, Bear.  Taking T or not is your road to travel, but if you do decide you want to do it, In my experience the different providers vary quite a lot at HBHC in terms of style.  I could probably get you some other names of folks you might click with better if you want to PM me.

About taking your shirt off all the time: as a post top surgery bday gift, one of my friends rigged up a button down shirt with little plastic windows attached with duct tape to show off my chest. I had been saying I was never gonna wear a shirt again, but then, you know, Chicago winters. It was hilarious.
