phalloplasty stage two post op advice

Hi. I've recently had stage 2 of the phalloplasty srs surgery (urtheral lengthening, hysterectomy,oophorectomy,vaginectomy, glands sculpturing,and a reservoir insert) and i was just wondering if anyone had any post op advice for me. Im 5 weeks post op now and have completed stage 1 ( radial artery phalloplasty with arm graft) in February 2014. Recently had surgery october the 25th with the london team.
I also wanted to help people out who are considering bottom surgery and any advice i can give i'm happy to help with any questions or anything. If anyone has already completed the full srs with the penile pump as well id be really grateful for there help or advice. As I'm just waiting on my final stage next February and would like to know some advice about the post operative information as after each operation i don't get a lot of advice or information. Its really about the fistulas and leaks ect.

Congratulations on getting through stage 1 and 2 so well.  I am pre any surgery and always interested in hearing how one of our brothers is doing with the surgery.  Your second stage sounds very involved to me so much was done.  Getting all internal parts out including the vagina, what is glands sculpturing?

Thank you. It's been a difficult time both physically and mentally but I'm finally near the end and will be completed by next year so just the waiting around which i know effects everyone of us even thought I'm 22 and i know people much older then me also wait such a long time for bottom surgery.

The glands sculpturing is when they shape the head of the penis to make it look more realistic so basically after stage one i had my phallus and one prosthetic testicle in place, then at stage two i had (all the stuff i said before )the glands sculpturing so the phallus looked like a circumcised penis and i am extremely proud of the results :)

I am 6 weeks post op 1st stage with the London Team and slowly recovering, not as fast as I thought... Some issues with the underside and a haematoma that refuses to go away...

How long were you in the hospital?
When do they remove the catheter?
Did you have any fistulas,leaks?
Where did they take the skin from for the urethra lengthening?

I agree not lot of information out there, so everything and anything you tell us is appreciated, really. Thank you.

thanks mate :) congratulations to you too for completing the first stage which is supposed to be the most difficult stage of 3/4 depending on what operation you've had and how many stages.

6 weeks is usually the time when you can move around a lot more and genuinely feel better in yourself. I remember i had a little opening underneath the phallus and id tried everything to get it to go away and it wouldn't so i kept calling st peters but they said i just need to wait for it to heal because different people take different times and eventually at about 8/9 weeks the little opening had healed. I didn't have any haematomas so i can't really help you there but all i can say is sometimes things take so much longer and if London aren't worried they you shouldn't be and if your concerned in anyway or things seem to be getting worse then again just either ring them or send an email. Theres nothing they can't do to help you and if you were going to have any severe complications then they usually happen within the first 3 weeks but still just keep an eye on things and if your worried ring them. I know its a mentally draining time and so many emotions and physically so hard but you will get through it and in a couple of months time you'll be looking forward to your next stage and everything will be more normal again. Just got to stay strong.

I was in hospital for a week monday to monday from feb the 10th at spire thames for the first operation.

For my second stage saturday the 25th of october
I had my phallus catheter  and my drain removed 1 week post op and my supra pubic ( abdominal) catheter removed at 3 weeks after my op date.
In terms of removing the supra pubic catheter basically depends on if you can urinate through the phallus and your passing a good flow of urine . I had and still have a fistula leak near the top where literally a drip of urine comes out when i go for a wee so i got told to just hold a tissue on it and put pressure and it should slowly heal up in a couple of weeks ( again its gonna take longer for me but I'm not worried). I also have two little stitches that haven't healed down the bottom I'm gonna give it another month because like i keep saying healing varies with different people. Ive been told not to worry about any leaks only if they get worse rather then better but I'm sure its a problem easily sorted

A little bit of pre warning as well about the supra pubic catheter it may give you bladder spasms which is like a having a constant stomach ache but i got some pills for it but again London didn't warn me so for three weeks i was in agony but as soon as the pills kicked in i was fine . Also water infections are very common and easily treated with antibiotics.

As for the uretheral lengthening i think they just did something to my original urethra. On my first op i had to push a catheter through my phallus once a month to keep my 'neo urthera'( i think this was used to make my new urethra well tie up my old one with my new one and pull it through my phallus) open until my next stage. I also had to keep flushing 60ml of water twice with a little syringe everyday ( which isn't painful just feels odd lol )   Have you been to back to harley street yet? if you haven't one of the nurse whoever you see will show you how to do this. It does get annoying and a little bit unnerving every month it comes round to pushing it through but you kind of get used to it and it just makes you want to get to stage two so much more lol.

If you've had the arm graft i remember being more stressed about the fact i couldn't use my arm and sleeping was always so uncomfortable but it will get so much easier i promise you :)

Jaybing, thanks for the information, your comments here are so appreciated I have read so much but to hear from someone just going through the surgeries so nice.  Your final description after gland sculpturing sounds like you have a very natural circumcised penis; I have always through I wanted a natural uncircumcised one but think that making one just adds more problems and I will be satisfied with a nice circumcised one. I see you already have the reservoir for the erection pump in place, must be easier to put it in place during stage 2 than wait for when the rest of the system is put in.  I want to get erection device when I have mine done, as it will be so nice to get an erection like a cis guy.  I have read about these devises and see that many older guys get them, but also some younger ones too, in there 20's and 30's.  This makes me think that I willn't be the only guy in there 20's with one. 

It must be so nice the look down and see and feel your a penis and you can even stand to pee with it. Are you getting feeling in it?  I know reading account of guys who got experimental surgeries 30 yrs ago had no feeling and could get them caught and injured in their zippers.  Did you have your enlarged clitoris placed inside your new penis?

Hi mm thanks anything i can do to advice people in my situation and help out with my experience i will.
I was considering not getting the erectile device because I've read that some guys can actually have sex without it but i guess in the long run i don't want to wait anymore and just say I'm not one of those guys and i can't have penetrative sex then id be put on a waiting list for the pump.
It's great being able to finally wee standing up and look down and just finally physically be me but its taking some getting used to lol as it is very different but in a really really good way. Back to the pump, a lot of cis guys have them due to erectile problems so to have the pump seems the right way for me.
I can feel my phallus especially at the tip actually that seems to be the most sensitive and more on the right side of it then the left but again ill have to wait and see what happens as the nerves connect again. To start with after stage one i couldn't feel much but after stage two i can feel a lot more and it is very sensitive. I had my clitoris hidden and put at the base of my phallus as you have an option to keep it in the same place but i wanted mine gone visibly. As I'm only 5 weeks post op I'm sure ill get more feeling over the next few months.

Thanks mate, I saw Sara at 4 weeks post op and email her pictures weekly. I do the flushing with water every day as well, although with only 20ml.  Hopefully by the end of next week the underside will heal completely.

So you go home with the supra pubic catheter still attached right? Any special after care for it? Thanks for the pills tip.

Hi jason glad i could help :)
I had to keep sending pictures of my arm graft to sara because it took about 7 and a half weeks to heal. Like i said different people have longer or shorter healing times but it will heal just keep an eye on it.
Yes, you have the supra pubic catheter in for 3 weeks post op and the drain and stent ( to keep your new urethra open through the phallus ) one week post op.
When you leave hospital they will give you a leg bag that you change every 3/5 days and you then get a delivery of day bags and night bags (which you have to change every night but the nurses will show you everything) they also provide some tape so it is attached to your leg so it doesn't pull which you will have to change as well because if it pulls the bag is heavy ( full of urine) then it pulls on your bladder so if your feeling a lot more discomfort just check that the tape isn't coming loose. The catheter is annoying when it comes to showering which you can't do until 7 days post op and also when you go to your nurse or whoever to get your drain and stent out ( one week post op) they will look at your dressings and change them etc. if they need so. I heal pretty quickly in terms of stitches but to be honest this op is a lot more internal with a lot more dissolvable stitching. Its just gone 5 weeks post op and I'm now getting discomfort and some spasms from my abdominal area which is probably my nerves connecting which usually takes 6 weeks on wards. Also my fistula has healed which took two weeks. Its really normal to have leaks and stitches not healing for even 3 months at the latest ( I'm still waiting on two stitches to heal). Also when you can bath then have salt baths they are the best thing for stitches and healing. Im having two a day and they seem to be working so theres a little heads up for your second stage.
