Hello guys,
I was wondering if any of you had undergone though SRS with Dr. Toby Meltzer. I am in needs of the CPT Procedure Codes that he uses for SRS. The reason I need this information is because since he is an Out-of-network provider with UnitedHealthCare which happens to be my Insurance Administrator needs me to file a Clinical GAP exception listing the codes of the procedures needed for my diagnosis 302.85 ICD9.
I have gotten in contact with Dr. Meltzer office and they simply told me that the GAP exception needs to be file by me when UHC told me it has to be file by him. UHC after all told me that if I wanted to file it myself I would need those codes in order for the to review it and approve that no other provider within a 30 miles radius could perform such procedures. So I am in desperately need of those codes. If any of you guys had surgery with Dr. Meltzer, could you be so kind and provide me with those codes?
