It depends on the situation. If its your regular physician, then yes, you should. Ask that it be either be kept out of your records or reworded in some way. Mine just put erectile dysfunction.
If you were to go into the ER with a cut that needed suturing, I wouldn't mention it.
As far as forms go, I no longer put down that I had all those surgeries. In a situation where you have had the surgeries within the past ten years, if you don't want to "out" yourself, you can say you had surgery for gynomastia or forearm graft for groin surgery. Chances are, they won't ask for the circumstances surrounding the surgeries.
