So my girlfriend changed her name legally about ten years ago. She has SSI card and drivers license everything is in her correct name. But, every year she files for taxes in her correct name, the tax return still comes addressed to her old name. Does anyone else have this problem or have any ideas how to fix this?

You mean the blank forms? I don't get them anymore. Call 1-800-TAX-FORM and tell them take you off the list or ask them how to change the name. I use turbotax or an accountant anyway and file electronically.

Your tax return should match what social security has. When your name is changed with Social Security they notify the IRS. Sometimes what will happen is the IRS will start 2 separate files under two different names even though they are the same Tax ID number. (This happen to someone I know.)

I just use TurboTax now myself. Simple, easy, quick.

I had that same problem... and to be honest, after a couple years I fixed it by gritting my teeth and signing up for direct deposit. The bank teller I had to deal with to cash the wrong-name check said it happened to her for years after she married, too.

I know that's not very reassuring. :( Sometimes the federal bureaucracy gets an idea in its database and will not be discouraged.
