Open rhinoplasty is done through an incision across the columella that allows the surgeon to freely roll up the skin of the nose. This way all the internal parts get fully exposed to be worked on, in opposition to the "closed" rhinoplasty that is done through the nostrils in a more "blind" way. It is very common in America. Dr. Ousterhout hasn't used anything but the open technique during his whole career. He has used it for every single case, and many other American surgeons do the same, but for some reason Latin American surgeons seem to be more reluctant to use it, like if they considered it suitable for just special occasions.
In my opinion the little scar that is left on the columella is well worth the possibility of your surgeon seeing what they are working on.

My surgeon did and open rhino, to be honest it simply is above the closed method and the scar is totally INVISIBLE, even 3 months post off which I am, and even in daylight, no makeup, it just isn't there, so it shouldn't really be a problem.

With Dr.Z's endoscopic approach he claims he can get aggressive with it.  Wondering if he does type 3 with the endoscopic technique?
