Transition in New Mexico

I'm just seeing if anyone can offer me any advice.
I currently live in Pennsylvania and I have a doctor that has prescribed spiro, but not E yet as I'm trying to get my blood pressure under control. So that's all well and good. Now the worrying part.
Moving to New Mexico at the beginning of November. Reason being that we are financially screwed right now with no income or prospects for a while. We have a place to live in NM and help getting there and a chance to get back on our feet. That's the good.
What has me worried is finding a doctor that will continue my hrt and allow me to continue where I'm leaving off in PA. I'm going to be in a really rural area and probably be broke for a while yet. Rural meaning silver city area.
I've looked online and can only find things in Albuquerque and maybe las cruces. If anyone has any advice or knows someone more local and friendly I would greatly appreciate that.
I know it's going to be hard, but I have no other options right now. Thank you.

You might can stay under the care of your doctor in PA, so to speak.  Just find a suitable physician that will work with them over the phone to monitor your situation and give them a detailed explanation of how to proceed with your treatment.

I say this because I know my neurologist and endocrinologist are talking about hormone treatments pertaining to my MS condition, so doctors will talk to each other to come up with treatment strategies.

Hmmmm. I wonder if they will do that?  I still have an appointment this month, so I'll bring that up with him then.
