If you're in the USA, by law, your doctor is supposed to keep everything confidential. If you are 18 your doctor can't even tell your parents. Don't worry too much about that part.
The changes on hormones are that your body shape and size will change, you may have some emotional changes and you coule experience some other side effects like dry skin and having to pee a lot depending on what medications you're prescribed. To me it's nothing really to be scared about.
Honestly if you're scared of the changes I would start by seeing a gender therapist who can help guide you through the whole process and even help you to decide whether you want to transition and how far you want to go.
