You are right: You probably wont start HRT that day. My first visit with the physician involved very standard or a generic interview such as asking about family health history, routines such as diet and exercising, and discussing the medical risks of HRT. I did have to take a blood test, but I did not take it that day because I had not fasted. If your initial visit is in the morning, then I would consider fasting in case you can take a blood test that day. My physician also gave me a general form, which I had to read and initialize to give consent on beginning HRT. It listed very generic medical risks, and mine also included some social risk [I was surprised this was included because I thought this would only be for my psychologist to worry about] (eg. changes will likely become noticeable to friends and family members, and it is not unusual for transgender people to lose their family and friends.) I think the form only listed very extreme ends of possibilities, so a lot of things on there will sound very bad and dangerous, but they need to assume that worst can happen with you and they are trying to cover their own behinds in case it does happen. It listed a lot of the perks I wanted too, such as describing breast growth and loss of muscle mass.
I would come with a list of questions. Write them down and knock them out one by one with your physician. You can tell them what you have heard and what you're expecting or hoping for. For example, does HRT cause your voice to change? Is there a general timeline? Your physician might be able to provide you with one. What's your general diet like and what should you change? Are you allowed to take certain vitamins such as Biotin and Fish Oil with your HRT medication? When should you call your physician and go to the emergency, or what is considered normal? It would be really nice to post up what you found so that others on a similar regime or beginning too might have an idea, but do recall that you cannot post your specific dosage (eg. 1 mg of blabla medication, but you can post I am taking low-dose blabla medication).
The physician might prescribe you very different dosages and even different medications compared to everyone else, so we might not be able to answer your question as well as your physician, and I am sure a lot of us are not medical professions. Your body is also different from all of us, so you are starting from a different point with different history.
