Most people in their 20s are either in school or working, or both. It sounds like you feel very stuck where you are. What I would do in your shoes:
1) Figure out where you would like to live. Ideally a state with trans legal protections, and a city with good public transportation and transgender healthcare available. Come up with 1-3 options that would work for you.
2) Research schools of all kinds in those cities - trade schools, community colleges, public and private colleges and universities, etc. Come up with a few different options that you think would work for you.
3) Fill out your FAFSA. If it's a trade school, contact them directly about financial aid, apprenticeship programs, or guaranteed work placement.
4) Apply to each school and wait to hear back. They will either send you a financial aid notice along with your acceptance letter, or shortly after. Go to whichever school offers you the most money relative to the cost of living in that city. Max out the loan options that are available to you to cover living expenses while you are in school.
5) Move. Apply for food stamps, disability, etc. You will have to update your FAFSA the following year to reflect this new income, but it shouldn't reduce your aid too much. The student loan lender will pay the school directly, and the school will cut you a check for any excess aid after the school year starts.
6) During the first week you are at school, go to the financial aid office and ask about work study positions. If you are broke, you qualify for work study. They should give you a list of on-campus positions that are work study eligible, and who you should contact about them. These would mostly be desk jobs in department offices. They are good to have on an otherwise blank resume and are generally not fast-paced.
Boom. You will have moved, gotten public assistance, and started over. As far as money to make the moving happen, ask your folks AFTER you've gotten into school. Let them know that you applied, got in, and want to start fresh in a new place. Tell them you're covered as far as school expenses but would appreciate help with moving expenses if you're not able to save for that yourself.
It is not impossible to do. You just have to break it down into manageable steps.
