Scalp hair: from vellus to terminal?

I'm tired of wearing wigs and being in constant conflict between having hair but itching and not having hair and looking much more masculine without it... but I'm very bald on top, meaningful hair left only on sides and back. I've stopped shaving and I'm trying to grow my hair and hopefully if it grows enough, maybe one day it will look okay with a hat that covers the upper part.

The following is an approximate simulation of how it will look, wearing a wig of my natural hair color and putting a hat above it.



It's not entirely representative and will probably look worse than this because:
- my hair in general is thinner compared to the wig
- the entire upper part of my head is not likely to grow hair, so that makes my total hair thinner compared to the wig
- my hair is straight, not curly like the wig

...but it's the closest approximation I have at the moment.

Before starting HRT I went to get examined by two hair doctors and they both told me there is no point doing hair treatments or transplants, because they said I'm totally barren up top and I don't have any excess thickness on the side and back to fill it up.

2.5 years of HRT later, and I don't know if this is new because of HRT or if this was there before, but though there are still very few terminal hairs left up top, I can see lots of vellus thin hairs up top. Is there any chance for these vellus hairs to be converted to terminal hairs? Is there anything I can do to help make that happen?

If I'm already on HRT and my T is very low, is there any point in taking DHT lowering drugs? (my endocrinologist says no, that there is no way I have DHT because I generally don't have T)

I'm not holding any hopes of it being realistically possible for me to get my hair back (became bald at age 21, found out I'm a woman only at age 32, started HRT at age 34) but as long as I'm growing my hair to try the hair + hat option, I'm wondering what I can do to give my hair a better chance to come back while I'm at it.

Is it theoretically possible for vellus hairs to become terminal hairs? Or if after 2.5 years of HRT they are still vellus hairs they will stay vellus hairs?

Should I go and get examined again? Maybe today the doctors will have a different diagnosis if previously I somehow didn't have these vellus hairs? (is that possible that I previously didn't have them and they grew back to vellus hair status due to HRT?)
