Nipple Sizing Question for DI

So my roommate had very, very large breasts before he got his top surgery and also REALLY large areolas and nipples.  Like super sized, apparently.  He said you could see his nipples through a bra, a shirt, and another shirt.  So when they did his surgery they had to resize it all and he was telling me how they cut across the nipple in an X shape and then basically remade it, which really freaked me out.  Is that the normal thing that they do or is it because his needed so much resizing?  I have small breasts (barely a B cup), small areolas, and small nipples.  Sure, maybe they are slightly bigger than an average guy's, but different guys have different sized nipples and, honestly, I am fine with mine the size they are.  So can they just place them where they are supposed to go without cutting through the nipple and reshaping it and stuff?  I like how my nipples look, and while his nipples look good they don't look as good as nipples that haven't been basically reconstructed, you know?  I am just trying to figure out some of this stuff, because I wasn't worried at all about having top surgery until my roomie started telling me all about his in great detail, some of which was rather terrifying, LOL. However, I am thinking mine may not be as complicated since I don't have DDD breasts to remove?  I think mine are just a little too large for keyhole, but I am hoping they can just leave my poor nipples as they are instead of slicing them up and sewing them back together in a vaguely nipple shaped way.

Wow, reading that back over this is a really weird question to be asking.   :laugh:

You can request how big you'd like them to be :) I didn't have big ones pre-op, but I disliked the size of them so much that I made a point to ask about it during my consult. I asked for nickel-sized nips and got nickel-sized nips. I think my surgeon said she typically did quarter-sized if folks didn't ask for smaller?

As for the rest of the question - peri or T-anchor technique would leave them attached and you could have some minor resizing done. But you would need to find a surgeon that does those techniques. If you're barely a B-cup and you don't have a lot of sagging, you definitely qualify for peri.

I would suggest finding a surgeon that does both peri and DI and getting their opinion. I know my surgeon was willing to do a combination of procedures on me to minimize scarring, but wouldn't guarantee that it would be 100% perfect in a single surgery (ie. I would probably have needed a revision), just do to the size being great but the skin quality being awful due to years of binding.

That makes sense, but what I really want to know is about the actual nipple itself--like the part that sticks out when you get cold. That was the part they apparently cut across in an X-shape to basically totally reconstruct. They resized it on him because it was so big, but while it looks generally nipple-like (it sticks out from the areola part) it's just not the same visually as normal nipples.  Mine really are pretty tiny, though!

I'm in the Dallas area, which is great because Raphael and Dulin are both known as really great surgeons.  I am getting my second letter tomorrow (my insurance requires two) so that I can get a consultation.  My breasts are pretty firm and the skin pretty tight I think--there is no stretching or anything like that--but, to be honest, I really don't know what level of "firmness" is required.  I haven't touched a whole lots of boobs other than my own over the years, so I don't know what the norm is when it comes to elasticity of breasts!

They aren't going to X cut them, that sounds like a one off thing for your friend. Usually they cut around the nipple and then cut it down a bit leaving it in its circle shape then they will cookie cutter the areola to make a new areola and then cut a little circle out of the middle and insert the cut down nipple. If yours are small enough the surgeon will probably just detach the whole nipple and areola together then reattach it after the boobs are flat.

I had absolutely nothing done to my nipples. And I really mean nothing. Apparently there was at least one good thing about my chesticles and it was my natural nipple placement. And size which makes it two. :D
Anyhow, if you're lucky enough with size and placement you don't even require nipple grafts. The surgeon just cut off the extra skin from the underside and removed the excess from the inside.

Awesome! Thanks guys that makes me feel a ton better. I honestly am fine with my nipple size so hopefully if they have to graft they can just detach the whole thing and put it back on without shaving down the nipple.  They are no bigger in size than the guys I have dated.  The areola is a little larger (just slightly above quarter size), but the nipple itself is really small.  I am really excited to get my second letter tomorrow and schedule a consultation with the surgeon!

I would ask your surgeon what they recommend. I'm guessing I am probably around a B as well and while still borderline I still qualified for peri because of elasticity. Measuring the mosser method is one way you can guess: http://www.trans-health.com/2014/mosser-method-ftm-top-surgery/ and also look up results on transbucket and from your surgeon to see what people looked like before who qualified.

Oh yeah, mine definitely look like the ones in the first photo of that Mosser scale thing--my nipples don't hang down at all  and it is not a very acute angle where my breast meets my chest.  Man I can't wait to get this letter and get a consultation!  I am tired of waiting.  :)

My actual nipples before my surgery were probably the size of a pinky finger tip. After surgery though, my left nipple doesn't stick out and it's basically flat, puffy around the areola. My right side however, has a nipple and it's about the size of a bb or a small bead. The areolas are oblong and slanted now. I'm happy with them but I guess since my left side was more swollen it caused the issue with the nip. It's a little depressing but hey nobody is perfect.
