Your best bet is to read your policy to confirm that there are no exclusions on it related to transgender surgery. After you confirm that, you would need to call them and see which surgeons are considered in network under your plan.
You'd then want to confirm that those surgeons do all of the procedures that you are interested in having done.
I personally don't know of any surgeons in CT. The closest geographically I can think of is the team at Mt. Sinai Hospital in NYC that is planning to start offering bottom surgery this fall.
