Take a look at this document which outlines the process in the UK (Specifically England) and I recommand you take a copy of it to your GP when you ask for a referral to a GIC.

Once you have your GIC choice, most have their own websites and often have their referral forms on there too, if so print it out and fill it in as best you can to make less work for your GP.
As for meds, GP's can prescribe bridging HRT but unless your not their first trans patient is extremely unlikely they will, there are some private routes available, quickest will likely be dr webberley at
gendergp.co.uk who will prescribe them under informed consent, if you want a more GIC like experience there is gendercare run by charing cross doctors, and the GIC like experience with them is starting to include the wait!
Either way be prepared for frustration but things do get there in the end.
