What am I doing wrong?

I can't figure out why people get such drastic results. I'm 18 months on E and Finesteride, a year on spiro and progesterone, I've been on Lupron since February, and I look almost the same as when I started. This is aside from growing my hair out, having a slightly thinner waist and my breasts growing out to make it look like I have gynecomastia. My levels are in good ranges according to everything that I can find online and my doctor as well.

It drives me crazy that it seems to work for other people who have been on it less time than me. How do other people actually get results with this?

Genetic differences mean we don't all respond the same way. Before surgery I was on HRT around 5 years and only developed a AA cup size. One girl had a C cup and most of the group had more than I did with less time. You have only  been on HRT a relatively short time and it's possible you still have more growth potential and it could take 3 years or more to know how you will develop.

Your results are probably more noticeable than you think... with seeing our same visage day in and day out, it can be hard to compare the results objectively.

You probably know too that a successful transition isn't just the result of taking pills in a bottle.  Being a woman is alot work for cis-women themselves!  Age is a pretty hefty factor in achieving pleasing outcomes too (idk your age tho).

HRT results are extremely variable... for some dramatic things happen quickly and for some only minor changes happen even in the long run.

It can be frustrating and disappointing when things don't go as one hopes but all we can do it learn to live with whatever hand we have been dealt.

In my case i did what what I could. Went to different endos, I had BA with SRS, I had
"the works" for FFS, did an experiment voice surgery, lost a lot of weight which made me more readable (so put it back on - well more than I should have)...

In the end I know i did all I could and it's Ok ... As the Stones said...
"You can't always get what you want, but if you try some time you find you get what you need"

If you think about teenage girls, you may get a sense of the huge variance in rates of development.  Across the 2-5 years of puberty, the differences between each teenager are significant.

As we go through a second puberty ourselves, we can take some solace in that even for cis girls, puberty development takes a long time and varies by quite a lot.
