Question for the people who take injections...

I have been on the "how do you feel" methodology of injections every 14 days.   I am considering going out of pocket for my blood tests for T/E for some imperical data.  So at what day/days would I want to have my levels checked?   
At what point did my life go Looney Tunes? How did it happen? Who's to blame?... Batman, that's who. Batman! It's always been Batman! Ruining my life, spoiling my fun! >:-)

My doctor prefers to see me at the very end or very beginning of a shot cycle. Since I do my shot on Saturdays, I typically get blood taken on a Friday. I do mine weekly though, so it may be different if you're on a larger interval.

I'm also weekly and we take levels the midpoint, and being relatively anal for details I try to be sure I'm at exactly 3.5days for measurements. I've assumed if biweekly works at all then weekly must deliver a very steady blood level and have been considering going to biweekly to see if I didn't actually prefer the presumably greater difference in levels.

To your question, I'd think definitely wait at least a couple of days before testing and I'd think the most useful numbers would be at 7,13 days.

I just saw a paper that compared two different estrogens to establish equivalence of pharmacokinetics and I believe it included the timing of peak post injection E level but I don't remember the numbers and only read the abstract.

Contraception. 1980 Apr;21(4):415-24.

"Pharmacokinetic properties of 3 natural estradiol esters, (estradiol cypionate, valerate, and benzoate), were assessed by assaying daily plasma levels of estradiol and estrone in groups of 10, 9, and 10 subjects, respectively, before and during 3 weeks of intramuscular administration"

"Estradiol and estrone levels both reached peak levels about 4 days after the injection of the cypionate ester and about 2 days after administration of the valerate and benzoate esters. Estrogens were elevated for the shortest time (4-5 days) with the benzoate ester, which was followed by the valerate (7-8) and the cypionate (about 11 days). After 2 weeks, no subjects showed elevated estradiol or estrone levels."

Most use valerate (including myself), a few use cypionate and rarely, benzoate.

I personally think there is no point in measuring levels as they fluctuate so much. I inject every 5 days and my levels went from 2,500 pg/ml to 1,500 pg/ml in just TWO DAYS.

There is no such thing as an ideal level, no way to establish it as there is so much cyclicity in natal females and so much variation in sensitivity in human beings to levels. Levels also do not appear to be a fair gauge of health risk as very high levels in natal women and men have shown to be quite safe as long as it is estradiol and usually, taken non-orally or produced endogenously.

I am also on Estrodial Valerate, 20mg/ml Delestrogen vial (not a dose...  just distinguishing which vial for all those on lookers).  So if I am interperating it correctly.... I get my high in 2 days which coinsides with my feeling of "ZOOM" and that I should have depleated the shot entirely in 7-8 days and I have been basically menopausal through the second week?  And by increasing the volume injected, I was only getting a larger averaged dose for the 8 days that it's in my system?

Metabolism differs from one individual to another but it seems that most do best on a 5, 7 or 10 day schedule. Often, 2 weeks is too long. If you feel menopausal before your next shot, determine which day you start feeling this way and share this with your doctor so interval can be adjusted accordingly.

My endo has me take 2 tests, 1 a day before and 1 a day after a shot. Right now (and for the past couple months) my shots are every 5 days.

I had the same problem when I first started injections. I had to switch from sublingual estradiol because I was on the max dose and my levels were at 68 and my endo wanted above 200. So we switched to the shot once weekly. We found after 5 days I was getting hot flashes and drained of energy. So he had me split the dose and inject every 3-4 days. That worked amazingly for me.

Well, just a quick update for the wonderful people who helped me with this dilema.  Since I am already on a higher dose than they'd like to prescribe, they've decided on their own to do my blood draws and check my end of cycle levels and mid-cycle levels.  They're conviced that I was not "doing so hot".  I did my end of cycle last week and I will have my midcycle on Friday.  Hopefully I will have good news in a week or so.  I guess from the conversation that they were planning on doing in months ago...  but for some reason never got around to ordering labs.  Ha ha ha ha ha....  Well, that one is kicked off into high gear now!
